On 06/15/2026, Licensing Program Analysts (LPA) Vivian Trinh, LPA Jessica Carreiro, and Regional Manager Claudia Guangorena conducted an announced, in-person relocation inspection for a new Large Child Care Home. Applicant is relocating from a previously licensed FCCH facility, 304313977, to a new facility, 304314809. Licensing staff met with applicant Cho, Sarah, who provided a tour of the home.

A review of the Facility Personnel Report Summary indicates adults (2) adults live in the home. The adult residents received their caregiver background check clearances as required by the Department. Per the applicant, there is one (1) minor living in the home.

Applicant was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

The applicant stated that they are not registered with any Resource Foster Care agency and that they do not hold a Resource foster parent license. The applicant was reminded to notify the licensing office of any changes.

The applicant is requesting a Large Family Childcare Home license. Per the applicant, operation hours will be Monday through Sunday, 3:00 AM- 2:30 AM. Care and supervision will be provided to children aged newborn (infant), two (2) months to 12 years old. For the purposes of licensing, an infant is defined as any child birth to 24 months of age. FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

All areas on Facility Sketch LIC999 were inspected, including but not limited to off-limits areas. The facility is a two-story home with 4 bedrooms, 3 ½ bathrooms, living room area, a dining room, a kitchen, a laundry room, a front yard (not fenced), a Yard/Patio (Wall/fenced), and a garage. This facility does not have a fireplace. The upstairs is currently barricaded and will be when children under (5) years old are present.

Based on the Facility Sketch LIC999 submitted, the areas off-limits to children and parents are as follows:

The Applicant has designated all of the 2nd floor, the garage, and the kitchen on the 1st floor as inaccessible. The applicant has placed child-proof safety gates around the kitchen, and baby gate on the 2nd floor.

Areas Designated for Day Care Activities: The applicant has designated the first-floor living room, dining room, and one first-floor bedroom as day care areas. Areas used by children were inspected for safety, comfort, cleanliness, telephone service (cell phones), ventilation, and heating. The designated childcare areas were observed to have age-appropriate toys. The children’s bathroom is located adjacent to the first-floor bedroom. The applicant has a small step tool in the children’s bathroom.

Detergents, cleaning compounds, medicines, sharp objects, and hazardous items that can pose a danger to children are inaccessible in areas designated for children, and an applicant has placed a baby safety gate at the kitchen entrance. The applicant stated that there is no poison on the premises, and Licensing staff did not observe any. The applicant was advised that poisons must be locked with a key or combination lock. The applicant stated that there are no weapons or firearms on the premises. The Licensing staff informed the applicant that when firearms are present, they must be locked and stored separately from the ammunition per CCR 102417(g)(4)(C).

Licensing staff verified that the applicant has a fire extinguisher type 2A-10BC and reminded the applicant that the fire extinguisher must be serviced annually or as often as necessary. The pressure gauge on the 2A-10BC fire extinguisher(s) indicates fully charged, as indicated on the service tag observed. Smoke and carbon monoxide detectors were tested and are operable.

OUTDOOR PLAY AREA:

The applicant designated the yard/patio as an outdoor daycare area. Outdoor play activities will be conducted in the Yard/patio. The Yard/patio is appropriately fenced. Licensing staff observed age-appropriate play equipment and a play area free of hazards.

Licensing staff observed an in-ground community pool approximately 118 steps from her front door. Applicant stated a key fob is needed to access the swimming pool area. Applicant stated they will not be taking the children to the community pool.



The applicant stated she will provide food for the children. The applicant was informed that if children bring food from home, their names must be labeled on their lunch bags, and the food must be refrigerated/stored properly.

The licensee understands that there shall be one crib or play yard for each infant who is unable to climb out of it.

The applicant stated that parents will provide linens and blankets for napping. The applicant stated that parents will provide diapers, lotion, wipes, and formula for the infants. Licensing staff advised the applicant to supervise infants at all times during diaper changes.

A records review indicates that the applicant completed the required Health and Safety Training, including the Nutrition and Lead Poisoning components. The applicant has current (EMSA-approved) Pediatric First Aid and Pediatric CPR certifications that expire on 2/3/2028. A first-aid kit is available in the kitchen at the facility.

Applicant Cho, Sarah, provided proof of control of property.

Licensing staff informed the applicant that in the absence of the licensee, a qualified adult must be present to supervise children. A qualified adult is an individual who has a valid and current Pediatric First Aid and CPR certification, TB clearance and immunization, and a valid criminal record clearance associated with the licensed facility. Annual fees must be paid by the due date, or a late fee shall be assessed and/or the License may be terminated.

This is a relocation inspection. LPA reviewed as a refresher the following with applicant:

· Inspection Authority: All adults living and working in the home shall be made of aware of the Department’s right to inspection authority, which includes but is not limited to the right to enter the house when children are being cared for, interview children and adults, and review documentation.

· H&S 1597.622: Commencing September 1, 2016, a person may be employed or volunteer at a family day care home if he or she has not been immunized against influenza, pertussis, and measles. The applicant has submitted proof of immunizations.

· Health and Safety Code 1596.7995: Beginning January 1, 2018, all licensed providers, applicants, directors, and employees must complete training as specified on mandated reporter duties. Training is available at www.mandatedreporterca.com. The applicant has completed the required mandated reporter training. Recertification is required every two years.

· Safe Sleep Regulations: Licensing staff discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep , as an additional resource. Licensing staff also informed applicant of the importance of checking for and removing recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/ , and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

· Incidental Medical Services (IMS):Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. During the exit interview, the applicant confirmed that there are no Registered Sex Offenders living in the facility and the LPA completed the RSO profile in FAS. On this 06/15/2026, the California Attorney General - Megan’s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility address. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross-reference of each address on record for all registered sex offenders against all CCLD facility addresses, based on information shared by the California DOJ.

Once licensed, the applicant is required to adhere to the terms and limitations stated on the license. A copy of this report and all other Licensing reports must be made available to the public for 3 years.

The Large Family Child Care Home was not in compliance with Title 22 Regulations at the time of inspection. The applicant is required to submit the following corrections to the licensing office by the due date of 6/19/2026.

1. Criminal record clearance transfer pending for one adult living in the facility.

In the event additional requirements are needed once a final review of the application is completed, the applicant will be notified. A license will be issued once all requirements are met.

Appeal rights were provided. An exit interview was conducted, and the report was reviewed with Sarah Cho.
