25 | Licensing Program Analyst (LPA) Odom conducted an unannounced Case Management visit. LPA met with Director Francesca Chemberlen to discuss the Lead Sampling Testing conducted on 06/25/22. LPA Odom ensured the Lead Sample Report is posted. Licensee was advised on 09/13/22 the Lead Sample Report was to be posted; LPA confirmed the Licensee posted the Sample Report.
Director stated the outlets with high levels of Lead are inoperable. Outlet G is a drinking fountain in Rm #5 that has been closed off by shutting off the water outlet and covering it with plastic bag. Outlet M is a drinking fountain in Rm #8 that has been closed off by shutting off the water outlet and covering it with plastic bag. Outlet O is a drinking fountain in Rm #9 that has been closed off by shutting off the water outlet and covering it with plastic bag. Outlet Q is a drinking fountain in Rm #10 that has been closed off by shutting off the water outlet and covering with plastic bag. Outlet T is a drinking fountain in Rm #11 that has been closed off by shutting of the water outlet and covering it with plastic bag. Director stated they plan to remediate the outlets. Source of drinking water is children’s water bottles brought from home and refilled by staff using pitchers with Arrowhead filtered water gallons.
Based on LPAs record reviews the following violation was observed and is being cited in accordance with California Code of Regulations Title 22, Division 12, Chapter 3, Section 101700.3 California Lead Action Level at Child Care Centers is being cited on the attached LIC 809D.
Exit interview conducted and report was reviewed with the Director Francesca Chemberlen. A notice of site visit was given and must remain posted for 30 days.
Appeal Rights were explained. The Licensee was provided a copy of appeal rights (LIC 9058 01/16) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. First level appeals should be sent to the regional manager to the address listed above. |
