time frame for C1 being left unattended was estimated 19 seconds. However, the video footage was not available for LPA's review due to time sensitivity.
LPA also interviewed C1 who was qualified for interview. C1 told LPA that C1 was left alone in the hallway behind closed classroom door (Preschool 1). C1 stated that S2 found C1 and took C1 back to the classroom. C1 stated S1 was inside the classroom. C1 stated C1 was walking slowly behind the group of children and S1 did not see C1.
LPA reviewed the internal name to face transition sheet dated 1/2/2025 around 12:00 pm. It showed at the time of incident there were 11 preschool children with S1.
Base on the self reported incident, staffs' interviews, C1's interview, and reviewing documents related to the incident, it was determined the facility failed to provide visual supervision of a child in care, therefore, California Code of Regulations, Title 22, Division 12, Section 1014229(a)(1)- Responsibility for Providing Care and Supervision is being cited on the attached LIC 809D. This is a repeat of same violation within 12 months. A civil penalty of 1000.00 was assessed today.
The facility representative, Sandra Johnson was informed that the “Notice of Site Visit” must be posted for 30 consecutive days. The “Notice of Site Visit” must be posted on or adjacent to the door. Failure to post will result in Civil Penalty of $100.00. LPA Malek informed the facility representative, Sandra Johnson that this report dated 1/9/2025 documents type A citation shall be posted for 30 consecutive days as there is immediate risks to the health, safety, or personal rights of children in care. Also, LPA Malek informed the facility representative, Sandra Johnson to provide a copy of this licensing report dated 1/9/2025 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report.
A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. The facility representative, Sandra Johnson was provided with a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days.
