25 | On June 4, 2024 , Licensing Program Analyst (LPA) Vivian Trinh conducted an unannounced visit due to a self-report Unusual Incident Report (UIR) received on 03/26/2024. The UIR stated that on March 25, 2024, during the transition from lunch to nap time, Child 1 (C1) was left alone in the Pre-K classroom for approximately 7 minutes. Staff 2 (S2) was the one who noticed that C1 was left alone in the Pre-K classroom without supervision.
This is a continuation of the investigation initiated on 04/12/2024. LPA met with Director MaryJoy Hilsabeck. A tour of the facility was conducted, and a census was taken. Observed at the time of the visit was a total 57 children with 11 staff.
A review of the Facility Personnel Report Summary on April 12,2024 indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.
During the inspection on 4/12/2024, LPA interviewed the Center Director, 3 staff, and parents involved in the incident. LPA also inspected the PreK classroom where C1 was left alone. LPA reviewed the Incident Report with the facility representative as well as the response of the facility.
During the interview, Director informed LPA incident happened on 03/25/2024 at approximately 12:25pm during the transition period from PreK classroom to TK classroom for napping. LPA interviewed S2 who stated on 3/25/24, after lunch time, S2 went into the Pre-K classroom to get more sheets and blanket and noticed C1 was in the classroom alone. S2 immediately took C1 to the director. The director and District manager reviewed the cameras and confirmed C1 was left alone for about 7 minutes. C1 was returned safely to the teacher by director who had a conversation with the staff involved.
