immediately document the injury. Staff reported the incident occurred in October 2025. Three staff members
were present and stated the child fell and hit the diaper-changing table. Management reported reviewing camera footage at the time to accurately complete the incident report and stated that all parties were notified. However, the incident report could not be located during this investigation.
Based on interviews with 13 infant/toddler staff members, 10 infant/toddler parents, review of diapering, feeding, and napping logs, and review of video footage for a specified date and time provided by the RP, the agency has investigated the complaint alleging 1) Staff handle day care children in an inappropriate manner, 2) Staff inappropriately restrains day care children, 3) Staff did not follow day care children's feeding schedules, 4) Staff do not adequately supervise day care children, 5) Staff engages in verbal altercations in the presence of day care children, and 6) Staff do not properly document incident reports, although the allegations may have happened or are valid, there is not a preponderance of evidence to prove that the alleged violations did or did not occur, therefore the allegations are UNSUBSTANTIATED.
A notice of site visit was given and must remain posted for 30 days.
