At 10:20 a.m., Licensing Program Analyst (LPA) Alanna Gontarek conducted an unannounced Annual Inspection. LPA met with Perla Arrieta, Director, who guided analyst on a tour of the facility indoors and outdoors. This is a Preschool program which consists of 3 classrooms; Pre-K Room, Preschool Room and the 2's Room. Facility operation hours are Monday to Friday from 6:30 AM to 6:30 PM. LPA observed staff wearing masks per Department of Public Health's current guidance. Census was taken. There were 41 children and 6 staff present upon arrival. The licensee is in the conditions, limitations, and capacity specified on the license.

PHYSICAL PLANT: All areas identified on the Facility Sketch, for the Preschool program, were inspected. The following was observed during the tour of facility: Disinfectants, cleaning solutions, medication, and other items that are dangerous to children, were inaccessible to children. Per Director, facility has no firearms or children requiring medication at this time. Per Director, medications will be stored in the office when received. Director states that there are no poisons on the premises. LPA advised Director to store poisons in a key or combination locked, in an off- limit area. Cleaning compounds were observed to be stored in the in an upper cabinet in each room, inaccessible to children in care. Furniture and equipment are in good condition, free of sharp, loose, or pointed parts. Based on observation, at 10:55 a.m., LPA observed the Emergency exit door, in the "Preschool Room," to not having a functioning door alarm. LPA pushed lever of the door open, and alarm did not sound. The Emergency door leads out into a shopping center and busy street. Pictures were taken. Director immediately closed room, and moved the children into the available/extra Early Preschool Room.Per Director, Fire Department came out today to inspect door and alarm. Per Fire Department, door needs to remain unlocked for emergency purposes. Per Director, will have children from Preschool room use the Early Preschool room until new alarm is installed. Per Director, will submit a picture of the new installed functioning alarm via email, and will conduct a video call tomorrow, 11/5/2021, showing alarm functions properly. LPA observed no bodies of water on the premises. The facility has a functioning carbon monoxide detector located in the hallway. LPA observed one smoke detector, meeting the standard, in each room. Director states isolation area is located in the office. Per Director, there is a mat and a fitted sheet stored in the office for ill child to use. Fire extinguisher last serviced on 12/7/2020. First Aid supplies are stored in each room and office.

Per Director, food is prepared on site; lunch and snacks are provided. All kitchen areas/food preparation areas and food storage areas are kept clean and are free of litter, rubbish and rodents and/or any other vermin. All storage containers for solid waste, including moveable bins shall have tight-fitting covers that are kept on, and in good repair. Trash cans used to discard food have tight fitting lids as well. Menus were posted for a month in advance where it is visible by the child's authorized representative to view. Per Director, facility uses CACFP food program. LPA observed snack and lunch menus posted. Snacks were reviewed for availability, quantity and appropriateness to children in care. The facility provides breakfast, lunch, and snacks in between meals.

There is drinking water available to children indoors by drinking fountain. The children's bathrooms are clean and sanitary. Children nap on cots, and bedding is stored individually and is taken home weekly to be washed by parents at end of week. LPA observed cots to be clean and in good repair. The facility met all posting requirement. The California Child Passenger Safety Law was posted by the entrance of the facility.

LPA inspected the outdoor play yard while children were outside playing. LPA observed readily available drinking water, via water faucets. Outdoor playground equipment is in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. At 11:05 a.m., LPA inspected outdoor yard. LPA observed a large tree, with roots sticking out from the ground, and dips in the dirt and grass. The uneven ground can become a tripping hazard. LPA advised Director to level the ground by removing tree roots, and filling dips/holes in the grass/dirt. All areas around or under high climbing equipment, slides, and similar equipment are cushioned with material that absorbs a fall. There is adequate shade in the play yard. The facility shall ensure that each child is never left unattended, and is under the direct visual supervision of a staff person at all times.

FACILITY RECORDS



There is at least one person trained in CPR and Pediatric First Aid present during this inspection. The child care center has three additional fully qualified teachers designated to act in the director's absence, and is on file. Educational background, transcripts, training, and/or experience for each staff present are on file and were reviewed. LPA observed all personnel to be in good health, and are physically and mentally capable of performing assigned tasks. LPA advised Director that personnel posing a threat to the health and safety of children shall be relieved of their duties. LPA observed most current on-the-job training regarding lesson planning and assessments, language development, family communication, and positive behavior on 10/4/2021. LPA observed staff to have current Mandated Reporter Training (AB1207) on file. LPA observed staff immunization records (Influenza, Measles, and Pertussis) on file for each employee.

Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

CHILDREN’S RECORDS

In review of children’s records, files contain information including, but not limited to the following: Name, address and telephone number of the child's authorized representative and of relatives or others who can assume responsibility for the child if the authorized representative cannot be reached when necessary.


Children's roster was reviewed and is current. Children present were signed in. Per Director, school uses the Pro Care application to sign in and out children, and children's authorized representatives use a kiosk to sign children in and out. Children present were signed in. LPA observed each child’s file to contain a medical assessment. Disaster drill log was available, last fire drill was conducted on 9/18/2021, and earthquake drill was conducted on 8/12/2021.


Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department.
