25 | Licensing Program Analyst (LPA), Patricia Duron conducted an unannounced Case Management visit for the purpose to discuss the Lead Sampling re-testing conducted on 11/16/22. LPA explained the reason for today’s visit when I met with Assistant Director, Monique Gonzales. LPA to census. LPA observed 15 infants in care.
Community Care Licensing was notified that lead water testing conducted at the facility on 11/16/22 failed the allowable limit for lead. The Purpose of today’s visit is to follow up with lead re-testing results of Action Level Exceedances (ALE). Facility re-tested outlet on 12/19/22 after flushing outlet for three weeks (11/28/22- 12/16/22) prior to re-test. Director will submit new re-testing result to LPA via email when Director receives results from A-Tech Consulting.
LPA observed on 12/21/22 the facility had posted the Lead Sample Report.
On 12/16/22 LPA spoke with Director, Perla Arrieta who stated the one outlet that had ALE over 5.5 had been made inaccessible to children. LPA observed outlet to inaccessible via lock on door plus outlet is inoperable via covering sink with black trash bags and a sign that states Do Not Use.
No deficiency was observed in the areas that were inspected, in California Code of Regulations, Title 22, Division 12 at the time of the visit.
Exit interview conducted with Assistant Director, Monique Gonzales. The report was reviewed and discussed. Appeal Rights were provided.All appeals must be in writing and received the Regional Office within 15 business days.The staff was informed that the "Notice of Site Visit" must be posted for 30 consecutive days. The Notice of Site Visit must be posted on or adjacent to the door. Failure to post will result in Civil Penalties of $100.00.
