25 | Licensing Program Analyst (LPA) Romy Castanon conducted an unannounced Case Management visit. LPA met with Assistant Director, Monique Gonzales to discuss the Lead Sampling Testing conducted on 08/25/22. Assistant Director was advised on 09/14/22 that the Lead Sample Report was to be posted. During visit, LPA observed facility staff post the Lead Sample Report.
Assistant Director stated the outlet C (179 ppb) with high levels of lead is inoperable. Outlet C is located inside the infant/toddler room restroom. The restroom is not used because the children enrolled in that room are not potty trained. LPA observed outlet C that tested with high levels of Lead inoperable via large black trash bag and duct tape wrapped all around. The children are using two outlets inside the infant/toddler room that do not exceed the level of lead to wash their hands. Facility provides water from those faucets/outlets and cups to bring to the outdoor play area for the children to use.
Assistant Director stated faucets were replaced, three week flushing begins 9/19/22, will retest and provide copies of the new test results to licensing office.
Based on LPAs record reviews the following violation was observed and is being cited in accordance with
California Code of Regulations Title 22, Division 12, Chapter 1, Subchaper 5 Section 101700.3(b)(1) California Lead Action Level at Child Cares on the attached LIC 809D.
Exit interview was conducted and report was reviewed with the facility representative Monique Gonzales. A notice of site visit was given and must remain posted for 30 days.
Appeal Rights were explained. Assistant Director was provided a copy of appeal rights (LIC 9058 01/16) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. First level appeals should be sent to the regional manager to the address listed above. |
