Licensing Program Analyst (LPA), Mahnaz (Nancy) Malek and Regional Manager, Claudia Guangorena conducted an onsite inspection for the purpose of an Annual inspection. Licensing staff met with Director Mandy Uieda. There were a total of 29 preschool children including toddlers from Toddler Option Program with 5 in two different classrooms and on the playground observed. The teacher child ratio was as follows: 2 staff with 9 toddlers from Toddler Option Program ages 18 months to 36 months old in Toddler Option Room (Little Bells Room), 2 staff with 12 preschool children in Preschool Room (Bell Academy Room), and one staff with 8 preschool children on the playground observed. The classrooms were inspected. There was a library area in the corner of the preschool room. In the side of the Toddler Option Room, there was a changing table within the arm's reach of a sink. The changing table had side bars at least 4 inch in height. The director was reminded to make sure children are not left unattended on the changing table during diaper changing.

A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.

LPAs toured of the facility inside and outside. The floor and yard plan (LIC 999) were checked. Facility hours are 8:00 a.m - 5:00 p.m., Monday through Friday. The parents sign their children in and out manually.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

compliance of staffing ratios. Preschool children and toddlers from Toddler Option Program nap on cots. Beddings for children are washed on a weekly basis and as needed by the facility.
The log for every 15 minutes check on napping children under 24 months old belong to toddlers of Toddler Option Program were available.

Floors, equipment, and furniture were clean and were observed to be in good repair and free of sharp edges. Drinking water is available to children indoors by water containers children bring from home and the facility fills them up at the facility if needed.

The facility has conducted an emergency drill within the past six months, last drill was in 4/27/2026.
The facility has Carbon Monoxide Detective which is operable.

The outdoor activity space was inspected for compliance. There was a separate playground for toddlers belong to Toddler Option Program. (18 months to 36 months old). There was also a separate playground for preschool children which were enclosed by a fence at least four feet in height. The surface of the outdoor activity space were well maintained and free of hazards except for two obvious areas on the surface that could be a potential of tripping hazard. (Two obvious spots were shown on the preschool surface). The surface of the outdoor activity area were Turf and commercial built in rubber under the climbing structures and all the surface. There is an exit gate opens to the outside area which was equipped with alarm device.

Drinking water in the outdoor activity space is provided by water containers which facility fills them up at the facility if needed. The outdoor equipment and toys were in good repair and free of sharp edges. Shade is provided. There are no bodies of water present at the facility.

Staff files were reviewed for staff present during the facility inspection on this date, 7 staff files were reviewed. They were in compliance. Children's records were reviewed, and there was a separate, complete and current record for each child. A random sample of 5 children's files were reviewed for documentation of the child’s name, address, and they were in compliance.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care
Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test as specified in Health and Safety Code Section 1597.16. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). LPA verified that the lead testing completed accordance to the Written Directives outlined in PIN 21-21.1-CCP. LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP on 2/17/2023. The results showed no lead exceedance.

Licensing staff discussed the safe sleep regulations with director and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep, as an additional resource. LPA also informed director, the importance of checking for and removing any recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

This facility provides Incidental Medical Services-IMS. Licensing staff reviewed storage of medication and equipment/supplies, and reviewed children’s personnel, and administrative records. The director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Based on observation today, the licensee did not comply with the section cited above in one count out of one count which poses a potential health, and safety risk to persons in care. Licensing staff observed there were two obvious spots on the surface of the cushioning materials on the preschool playground not in repair which could pose tripping hazard.
