32 | On 08/01/2022, 08/10/2022, LPA Nguyen interviewed 5 staff members and no disclosures were made by any staff members regarding the above allegations. All interviewed staff members denied of witnessing the water fountain being dirty. Interviewed staff members indicated that the facility staff members check the children’s temperature daily. If the children become sick or look sick, the facility will call parents and complete the reports. To prevent the head lice infestation, the facility sends the infected children home. These children could only come back 24 hours later after the treatment. The staff would check the children to make sure the children do not have any nits or headlice prior to allowing the children to be back in the classrooms. All interviewed staff stated when there is a head lice infestation, the facility will: disinfect the hard surface area, clean the carpet with Lysol, and vacuum it, send the children’ belongs (such as blanket, pillow, sheets) home so the parents can wash it, and notify the parents about the infestation.
During the inspection on 08/01/2022, LPA Nguyen observed the water fountain and the water dispenser at the facility were cleaned. LPA Nguyen did not observe any puddle sitting on the water fountain.
During the investigation, LPA Nguyen interviewed 4 children, no disclosures were made by any children. All interviewed children stated they like their teachers and their teachers check their temperature daily. The interviewed children also stated they have not observing the water fountain being dirty. LPA Nguyen contacted five parents by phone and was able to interview three parents. All interviewed parents stated they did not have any concern with facility.
Based on the information gathered from LPAs' interviews, observation, and reviewing records, there is insufficient evidence to corroborate the allegation of Staff did not prevent the spread of a communicable disease, Staff are not doing daily inspections for illness, and Facility water fountain is dirty. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged Staff did not prevent the spread of a communicable disease; Staff are not doing daily inspections for illness; Facility water fountain is dirty did or did not occur, therefore the allegations are UNSUBSTANTIATED.
Exit interview was conducted. The Notice of Site Visit was posted. Appeal Rights was explained. A copy of appeal rights (LIC 9058 1/16) was provided. First level appeal is to Regional Manager, address is above on the report.
