On 06/04/2026, Licensing Program Analyst (LPA) D. Valdez Santana made an unannounced visit to St. Norbert Catholic School Children’s Center for the purpose to deliver findings of a complaint that was received. Upon arrival, LPA was met by Interim Director, Maria Mills-Phillips who was explained the reason for today’s visit. LPA toured the facility; census was taken in individual classrooms, there were 28 preschool-age children and 4 staff at the time of inspection. Interim Director stated they currently have 44 children enrolled in the preschool.
A review of the Facility Personnel Report Summary conducted on today’s date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.
On 03/27/2026, the Orange County Child Care Office received a complaint alleging: (1) Staff does not ensure facility operates in ratio. The Reporting Party (RP) stated during drop off on Mondays and Wednesdays with occasional Thursdays, usually Staff #2 (S2) is alone in the classroom, supervising between 12-17 children. Page 1 of 2.

During the course of investigation, LPA interviewed 5 staff, 5 children and 1 parent.

During staff interviews, Staff #1 (S1) and Staff #5 (S5) both denied ever being out of ratio. Staff #2 (S2) disclosed being over ratio a lot of times in the morning in between 7:45-8am, it can be up to 20 kids. S1 is out greeting parents. Staff #3 (S3) stated S3’s classroom has not gone over ratio but yes, S2’s classroom has been over ratio because S1 was at the talent show for the upper grades and for about 10 minutes they were out of ratio, that was more of an emergency situation. Staff #4 (S4) disclosed leaving S2 out of ratio, when needing to use the restroom or make copies, S4 has stepped out for a bit. Five (5) out of five (5) children interviewed did not make any disclosures. One (1) out of one (1) interviewed parents stated they had no concerns about the facility and did not disclose any information related to the allegation. Additionally, LPA obtained children’s sign/out records and staff Timecards for March 2026. LPA reviewed the documentation and observed that the facility was operating out of ratio on at least one date.

Based on interviews conducted, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. California Code of Regulations, Title 22, Division 12, Chapter 1 Section 101216.3(a) Teacher-Child Ratio, Type A is being cited on the attached LIC 9099D. Please refer to attached 9099D for documentation of deficiencies.

LPA Valdez Santana informed Interim Director, Maria Mills-Phillips, that this report dated 06/04/2026 documents one Type A citation which shall be posted for 30 consecutive days as there are immediate risk to the health, safety, or personal rights of children in care.



Also, LPA Valdez Santana informed the Interim Director to provide a copy of this licensing report dated 06/04/2026 that documents any Type A citations to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

Exit interview was conducted. The Notice of Site Visit was posted. Appeal Rights were explained. A copy of appeal rights (LIC 9058 1/16) was provided and their signatures on this form acknowledges receipt of these rights. First level appeal is to Regional Manager, address is above on the report.

show on 03/11/26 there were 13 preschool children with 1 staff for about 2 minutes. S4 disclosed walking out of the classroom often to make copies or for a bathroom break leaving S2 alone with the children. This poses an immediate health and safety risk to the children in care.

During the course of investigation, LPA interviewed 5 staff members, 1 parent and 5 children.

During staff interviews, Five (5) out of five (5) staff denied that staff do not provide first aid to children in care. Staff #1 (S1) stated, the procedures for when a child gets hurt is staff usually take care of the child, treat the injury, make sure they are okay. Staff then write the “ouch” report and it’s given to parents. Sometimes staff call parents and inform them of the incident. S1 stated S2 and S3 have taught the children to help one another if one gets hurt but the staff are aware of the child falling, the staff is present and they teach them to help out. Staff #2 (S2) stated the procedures for when a child gets hurt depends on the injury, if it’s minor, staff clean it up and put a band-aid but if its serious, staff treat the wound and write the ouch report. Parents are called and they have to sign the report and so does the director. S2 stated yes, the children sometimes help one another by giving a wet paper towel when someone gets hurt, they see staff treat wounds so they will get paper towels to help but staff treat them. It has also happened where a child may say “I fell and so and so brought me a paper towel” and staff will check the child to make sure they are okay. Staff #3 (S3) stated the procedures for when a child gets hurt depends on the injury, sometimes the children will help get paper towels. Staff administer first aid and if its serious, staff call the office and they call the parents. When S3 gets back to class, S3 fills out the ouch report. If the injury is on the face parents are called. Sometimes we will see a child bring a wet paper towel to a child that fell, they do that to help out one another but as far as ice or putting a band-aid no, staff do that. Staff do teach them to help like Jesus because we are a Catholic school, we teach them to do like Jesus but only paper towels. Staff #4 (S4) and Staff #5 (S5) stated teachers treat the injuries and write ouch reports for parents and children just help with getting wet paper towels as a way to teach them to be helpful. Five (5) out of five (5) children stated staff take care of children’s injuries. Four (4) out of five (5) children stated children bring wet paper towels when friends get hurt to be kind and helpful. One (1) out of one (1) interviewed parents stated they had no concerns about the facility and did not disclose any information related to the allegation.

Based on LPA’s interviews and record review, the preponderance of evidence has been not met; therefore, the allegation of Staff did not provide first aid to the children in care, violation did or did not occur; therefore, the allegation is UNSUBSTANTIATED.
