On 04/24/24, the RP stated that the facility does not have a director.
On 4/29/24, LPA interviewed four (4) staff. 3 staff stated they have not seen the director, Dawn Silva or Melissa Wojick in the facility in the last few months. Staff 2 (S2) stated, S2 last saw Silva in January 2024. S2 stated, recently, Site Supervisor Alicia Flores stepped in to be the facility director. Staff 1 (S1) stated Wojick moved to the other location and Silva is in charge. S1 also stated Silva only comes on Wednesdays.
LPA observed director, Wojick not being present from previous licensing visits conducted in March, April and May 2024 and have not met Silva in the facility from all licensing visits. S1 stated, Silva is in the facility once in awhile.
On 4/23/24 the facility submitted director’s packet for Alicia Flores; However, the facility was without a director from approximately February 1, 2024 to April 23, 2024.
On 5/20/2024, the LPA interviewed twelve (12) parents. The LPA reached four (4) parents, none of the parents interviewed disclosed any information that could support the allegation.
On 5/22/24 LPA reviewed 1 staff file for director in training. Documents are incomplete at the time of inspection.
Based on observation, inspection, interviews conducted, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. See LIC9099D 1 Type B deficiency cited.
An exit interview was conducted, and the report was reviewed with Director Alicia Flores. Notice of Site Visit was posted during the visit. The director was informed that the notice of the site visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100.
Appeal Rights and deficiency were explained. The Licensee was provided a copy of appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. First level appeals should be sent to the regional manager to the address listed above.
