32 | **This is an Amended Report**During the staff interviews, Staff 1 (S1), Staff 3 (S3), disclosed that they have been over ratio. S3, and S5 disclosed that children are often shuffled into different rooms because they are trying to maintain ratios and subsequently comingle the different age groups. S1 admitted that although the staff schedules are staggered to meet ratio needs but acknowledged that staggering aides is what is needed to meet any staff-child ratio needs. S3 stated they are not usually over ratio but it has happened with 1-2 children over ratio. S3 has had up to 15 children and when that happens, S3 send one to preschool.
3 of 4 children interviewed disclosed there are often preschool children in the school-age classroom. Child #1 stated there are 3- and 4-year-olds in the classroom with the school-age kids all time both inside the classroom and outside. Child #3 (C3) and Child #4 (C4) disclosed that there is at least one preschool child placed with the school-age children. C4 stated Child #5 (C5) is placed in the school-age classroom since 3pm snack time.
3 of 6 parents were interviewed, parents had no issues or concerns with the daycare.
Based on interviews conducted, file review and observation, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. California Code of Regulations, Title 22, Division 12, Chapter 1 Section 101516.5(b)(1) Teacher-Child Ratio is being cited on the attached LIC 9099D. Please refer to attached 9099D for documentation of deficiencies.
Exit interview was conducted. The Notice of Site Visit was posted. Appeal Rights was explained. A copy of appeal rights (LIC 9058 1/16) was provided and their signatures on this form acknowledges receipt of these rights. First level appeal is to Regional Manager, address is above on the report.
LPA Valdez Santana informed Director, Melissa Wojcik, that this report dated 9/7/23 documents one Type A citation which shall be posted for 30 consecutive days as there are immediate risk to the health, safety, or personal rights of children in care.
Also, LPA Valdez Santana informed the Director to provide a copy of this licensing report dated 9/7/23 that documents any Type A citations to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. Page 2 of 2. End of Report. |
