This is Amended version of original report dated on 2/7/25.
Licensing Program Analyst (LPA) Patricia Duron conducted an unannounced complaint visit to deliver the complaint findings for the above allegations. This is a continuation of the investigation initiated on 12/18/24, LPA met with Director, Jennifer House and informed the director of the purpose of the visit. The director and LPA observed classrooms and census was taken. The overall census observed was 1 staff with 0 children.
A review of staff records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.
On 12/13/24 a complaint was filed with the Licensing office alleging (1) Staff do not ensure rooms are clean and sanitary (2) Facility not providing care and supervision to meet children's needs (3) Ratio
During the course of investigation, LPA interviewed 4 staff members, 5 children, 4 parents, and reviewed records.

Regarding allegation (1) staff members have not witnessed staff do not ensure rooms are clean and sanitary.

LPA interviewed 4 staff members, 4 out of 4 staff members have not witnessed staff not ensuring rooms are clean and sanitary. Staff #1 (S1) stated staff clean and wipe tables about 4 times a day. Staff clean tables before and after children eat. Staff sweep after every meal and mop at the end of the day. The bathrooms are clean daily and after every use if needed. Staff #2 (S2) stated, staff clean every day, about 3 or 4 times a day. Staff are always cleaning. The closing staff clean at the end of the day to make sure it's ready for the next day. Each room is responsible to keep their classrooms clean and organized.

Regarding allegation (2) Facility not providing care and supervision to meet children's needs

LPA interviewed 3 staff members, 3 out of 3 staff members stated they have not witnessed facility or staff not providing care and supervision to meet children's needs. Staff #1 (S1) stated, staff are always supervising the children, and providing activities for them, and staff make sure children are treating each other well and following rules to stay safe.

Regarding allegation (3) Ratio

LPA interviewed 3 staff members, 3 out of 3 staff members stated they have not witnessed facility or staff being out of ratio. LPA reviewed children’s sign in sheets and staff timecards from December 2, 2024 through December 13, 2024, and confirmed the facility was in ratio during the weeks of December 2 – 6, 2024 and December 9– 13, 2024.



During the investigation dated 12/18/24, LPA observed the school age classroom, LPA observed children’s toys, materials, and books were placed on shelves and in containers. LPA observed school age bathrooms, the children's bathrooms were observed to be clean and sanitary at the time of visit. LPA observed disinfects used by staff to sanitize classroom stored and out of reach to children.

LPA Duron interviewed 5 children. All 5 interviewed children made no disclosures regarding the above allegations.

LPA Duron interviewed 4 parents. All interviewed parents stated they did not have any concern with facility.



Based on the information gathered from LPAs’ interviews, observation, and the reviewing of records, there are insufficient evidence to corroborate the allegations: Staff do not ensure rooms are clean and sanitary and staff do not have the required qualifications. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the allegations did or did not occur in the day care facility, therefore the allegation is UNSUBSTANTIATED.

An exit interview was completed. The report was reviewed and discussed. Appeal Rights were provided. The facility representative was provided a copy of their appeal rights and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Licensing office within 15 business days. Any proposed changes to the physical plant, including telephone number, shall be
immediately reported to the Department.

The facility representative was informed that the “Notice of Site Visit” must be posted for 30 consecutive days. Failure to post will result in Civil Penalties of $100.00. The “Notice of Site Visit” must be posted on or adjacent to the door.
