32 | During observations conducted on 09/30/25, LPA V. Trinh witnessed another staff, Staff #2 (S2) handled children in a rough manner and addressed the concern with staff.
During the staff interviews, Staff 5 (S5) confirmed that they reviewed the camera footage of the alleged incident 9/25/25, in which they observed S1 handling C1 roughly while transitioning from the playground to the classroom. Additionally, S5 stated that on 9/25/25, S5 met with S1 to address concern and issued a warning letter for S1 regarding handling children in a rough manner. In the meeting, they shared various techniques with S1 and reminded S1 of each child's needs. On 10/22/25, LPA V. Trinh called five (5) parents requesting an interview. The parents interviewed did not express any concerns regarding the allegations.
LPA V. Trinh reviewed the Police Report on 9/25/25, which stated that on September 24, 2025, Camera footage showed witness S1 and C1 outside room #9 at school. S1 grabbed C1's wrists, lifted C1 slightly, and moved C1 to a nearby wall. S1 then placed C1 on the ground, facing S1, briefly touched C1's shoulders and cheeks to get C1’s attention, and appeared to be communicating with C1 without causing harm. Police Officer did not observe any visible injuries on C1's wrists.
Based on LPA V. Trinh interviews and record review, and police review record, the following violation of the California Code of Regulations, Title 22; Division 12, Section 101223(a)(2) Personal Rights was observed and cited on the complaint investigation report LIC 9099 dated 12/12/2025.
Exit interview was conducted with the Director Kathy La. Notice of Site Visit was posted during the visit. The director was informed that the notice of site visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100. The Director was provided with a copy of their appeal rights (LIC 9058), and their signature on this form acknowledges receipt of these rights.
