After inspecting the slide area and finding no obvious hazards, Staff #2 (S2) notified the administrator around 2:40 pm. Owner/Licensee Katelyn Le then assessed C1 and reviewed the playground security footage, which appeared to show a minor fall; however, because the camera angle partially obstructed the incident, the incident could not be clearly viewed in its entirety. Around 5:20pm, Owner/Licensee Katelyn Le notified C1’s parents, who picked up C1. According to Child 1's authorized representative, Child 1 sustained a fracture and is wearing a cast/splint. On July 23, 2026, Child 1 returned to school with medical restrictions.
During the course of the investigation, LPA interviewed two (2) staff members and two (2) preschool child, obtained a copy of the children’s roster (LIC 9040), and C1’s and C2’s record and incident report, LIC 500 Personnel Report, Parents Handbook, Doctor’s note with restriction for C1, playground map with where the teachers are standing on the day and time of the incident, and Video footage 7/22/2026.
LPA conducted an inspection on 08/03/2026 and conducted (3) staff interviews and interviewed two (2) children. Staff #1 stated that they witnessed Child #1 (C1) slip on a rubber mat while playing on the slides during lunch recess and land on his arm; they assisted C1 with first aid and an ice pack. The LPA also interviewed C1, During inspection on 08/03/2026, the LPA inspected the playground to ensure the facility has sufficient cushioning (synthetic grass with padding under the climbing structures) to meet safety requirements. During a subsequent video review of footage from 07/22/2026, the LPA did not observe the fall itself but did observe staff member S1 walking over to assist child C1 after C1 fell in that direction.
In the areas that were evaluated, no deficiencies were observed in the California Code of Regulations, Title 22, Division 12, at the time of the visit.
An exit interview was completed. The report was reviewed and discussed. Appeal Rights were discussed. The Director Kathy La was provided a copy of their appeal rights and their signature on this form, receipt of these rights. The Director Kathy La was informed that the 'Notice of Site Visit' must be posted for 30 consecutive days.
