LPA interviewed staff # 1 who said they were hired as of 8/15/2024. They will be the new director. S1 was supervising 8 preschool children. S1 stated among the eight children child # 1 is over 2 years old who belonged to Toddler Option Component. C1 is with the preschool children now. The facility has combined the Toddler Option Program with preschool children. LPA reviewed the children's files and found out C1 is enrolled in Toddler Option Program. LPA interviewed three staff in the infant room as well. Staff stated 3 children belong to Toddler Option have been combined with infant/toddler program. Staff stated they had four children in the Toddler Option Program enrolled with one staff. Recently, few staff left the facility and they had to combine few children with infant/toddler program and one child combined with preschool program. S5 stated there is one staff for Mondays, Wednesdays, Fridays for Toddler Option Program and they are in the process of hiring another staff for other days to cover the Toddler Option Program. S5 admitted they did not report to licensing that a new director will be on site after the former director left on 8/16/2024. S5 stated they would send the director's packet for the new director to the office.
Today, during the course of interviews with 5 staff, reviewing children's files, their birthdays, documentation of enrollment in Toddler Option Program, and observation of LPA that a child (C1) is enrolled in Toddler Option and has been commingled with preschool children, it was determined the preponderance of evidence standard have been met, the allegations of "Commingling, and "does not have a director." are found to be SUBSTANTIATED.

The facility representative, Melinda Marchese, was informed that the “Notice of Site Visit” must be posted for 30 consecutive days. The “Notice of Site Visit” must be posted on or adjacent to the door. Failure to post will result in Civil Penalties of $100.00.
