25 | Licensing Program Analyst (LPA) P Rivas conducted an unannounced annual inspection and was met by Mr. Ranman Jayasekara who did not allow entrance into facility. LPA explained purpose for visit but was still not allowed entrance.
Licensee stated he did not want to allow LPA Rivas entry as his appeal dated 09/14/22 & follow up letter dated 03/31/23 under 304370871 had not received a response. As per licensee's statement.
The following deficiency is cited under the Health & Safety Code 1596.852 Any duly authorized officer, employee or agent of the department may, upon presentation of proper identification, enter and inspect any place providing personal care, supervision, and services at any time, with or without advance notice, to secure compliance with, or to prevent a violation of this act or the regulations adopted by the department pursuant to the act.
LPA Rivas informed Ranmal Jayasekara, Adm/Owner that this report dated 07/26/2023 document(s) one Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care.
Also, LPA Rivas informed to provide a copy of this licensing report dated 07/26/2023 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
