During the course of the investigation, additional allegations regarding (3) Staff handled child in a rough manner, (4) The facility is not kept clean and orderly, (5) Staff do not provide children with adequate supervision, (6) facility does not report incidents in a timely manner were discovered.
During the investigation, LPA Garcia interviewed staff, reporting party, and children’s authorized representatives, obtained facility correspondence, employee handbook, family handbook, incident reports from the month of February and sleep logs from the month of February through March, and children’s roster.
All staff interviewed stated they have never staff thrown toys at a daycare child, left daycare child crying for a period of time, handled child in a rough manner, they always provide children with adequate supervision and report incidents in a timely manner. All staff stated the facility is cleaned throughout the day by staff and a third party company cleans every night. All staff stated they report incidents via the app, or an incident report and the director is available to step into the classrooms as well.
Based on record review, the sleep logs are being conducted every 15 minutes and in compliance. Incident reports are being completed and provided to children's authorized representative in a timely manner.
Throughout the investigation, LPA interviewed children’s authorized representatives. Representatives interviewed shared feedback regarding their enrollment experience, there was not evidence to corroborate the allegations.
Based on interviews and record review, the allegations that: (1) Staff threw toys at daycare child, (2) Staff left daycare child crying for a period of time, (3) Staff handled child in a rough manner, (4) The facility is not kept clean and orderly, (5) Staff do not provide children with adequate supervision, (6) facility does not report incidents in a timely manner are unsubstantiated. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are unsubstantiated.
Exit interview conducted and report was reviewed with the Director, Cynthia Gough. No deficiencies cited. A notice of site visit was given and must remain posted for 30 days. |
