Licensing Program Analyst (LPA) P Rivas conducted an unannounced complaint visit to investigate the above allegation. Upon arrival LPA met with Elaina Estrada, Director who assisted LPA with investigation.

The investigation consisted of interview with Director and physical plant tour. At 10:45 am LPA toured Pre-school (Room #3), Pre-K(Room #4), Kinder (Room #5 ), outside play yards( infant, pre school and kinder) and kitchen.
The investigation found Preschool classroom has changing table and diaper trash can. Upon entrance LPA did not note any odor. LPA viewed trash can with lid, in changing area. Interview with Director indicates that the diaper trash can is emptied twice a day, once after lunch and once after facility closes. After lunch diaper trash can is emptied by teachers, after facility closes it is emptied by cleaning service. Director reported 16 of 24 children need diapers changed. Diapers are changed every 2 hours or more often as needed. Furthermore, interview with Director indicates that cleaning services comes in after closing hours and duties include but not limited to; clean and sanitize restroom counters & sinks..., clean & sanitize toilet bowls, seats & urinals, empty trash bins and spot clean as needed, mop & disinfect restrooms, disinfect door handles & switches, pour clean water (or mixed bleach) down into floor drains in restrooms to prevent odors...

Licensing Program Analyst (LPA) P Rivas conducted an unannounced complaint visit to investigate the above allegations. Upon arrival LPA met with Elaina Estrada, Director who assisted LPA with investigation.

The investigation consisted of interview with Director and physical plant tour. At 10:45 am LPA toured Pre-school (Room #3), Pre-K(Room #4), Kinder (Room #5 ), outside play yards( infant, pre school and kinder) and kitchen.
In reference to the allegation that garbage cans are dirty the investigation found; LPA noted three large garbage cans outside on play yards and two large garbage cans inside kitchen. There were various smaller trash cans within each classroom. During tour of facility LPA noted the two large trash cans in the kitchen did not initially have a lid but Asst Director Sylvia Carillo quickly placed lids onto trash cans. LPA observed three trash cans one on each play yard. From 11:50am to 11:52 am LPA Rivas took pictures of outside trash cans. The trash cans on the play yards were dirty and one had a broken lid, all three had some form of decaying waste, such as a banana peel, snack food. LPA was able to corroborate allegation.

In Reference to the allegation that there are insects in the facility; the investigation found; At 11:04am LPA toured Preschool (Room #3) and noted on closed off water faucet, there is a red cup taped onto faucet so as to prevent children from drinking from faucet. There were dead insects on tape. At 11:30am LPA noted live insects in kitchen, in the kitchen sink and where the bananas were stored next to sink. The insects appeared to be flying gnats(small winged insects usually flying in groups). LPA photographed gnats in kitchen sink.

Based on LPAs observations the preponderance of evidence standard has been met, therefore the above allegations are found to be substantiated. California Code of Regulations, Title 22, Division 12, and Chapter 1 are being cited on the attached LIC 9099D


An exit Interview was conducted. Report was reviewed and discussed. The director was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. The Notice of Site Visit was posted. Director was informed that the notice of site visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100. LPA informed the licensee of how to access regulations and forms from CCLD websites: www.cdss.ca.gov

Per discussion with Director, she will have kitchen deep cleaned, will store all fruits propertly, will have pest control add gnats as part of service , will re tape cups covering water faucets in classrooms.
