During the investigation, LPA Nelson observed at the daycare center during normal operating hours. The two-year-old classroom was observed during class time on 04/22/2021 at 3:30 PM, 05/24/2021 at 2:00 pm and on 07/15/21 at 9:00 am and 11:30 a.m. LPA Nelson conducted interviews with the center director, LPA interviewed three teachers, four parents and four children were interviewed.
Interviews with parents were conducted, there was no disclosures made. The parent in question could not confirm the exact number of children in the room rather one teacher was observed from the doorway. Interviews with director confirmed that staffing ratios are always (1:12) in the preschool classroom. All three of the teachers did not disclose any issues of being out of ratio while working with their students. LPA Nelson conducted a record reviews of the sign in and out sheets from April 2021 to June of 2021 and no issues were noted during that record audit. Staff schedules were also reviewed from April to June 2021 as staffing ratios were in place during center operating hours in the designated preschool classrooms.
Interviews were conducted with the parent in question and the director. The parent in question stated that her child was picked up in soiled diapers but could not recall the date of the alleged occurrence. The director stated that the child in question would not have been permitted to go home with an unchanged diaper or a unkept face. The toddler teacher interviewed stated that she was not informed by the parent of any diaper rash issues. The toddler teacher makes sure when children come in from playing or after eating that they help the children wash up when transitioning from various activities throughout the day. The teacher did not observe the child in question face or hands being unkept or unsanitary. None of the parents made any disclosures about the sanitary issues when picking their children up from preschool. Interviews were conducted with the children who all stated that they wash their hands after playing outside.
