The allegation stating a Teacher left children unattended in classroom was investigated. Based on LPA Corral Staff and Children interviews there is not enough evidence to substantiate the allegation, therefore, the allegation is found to be Unsubstantiated. This agency has investigated the complaint alleging a Teacher left children unattended in classroom although the allegation may have happened or is valid, there is not enough preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is Unsubstantiated.
The Allegation stating the Facility staff do not provide a safe and comfortable environment was also investigated. Based on LPA Corral Staff and Children interviews, there is not enough evidence to substantiate the allegation, therefore, the allegation is found to be Unsubstantiated. This agency has investigated the complaint alleging the Facility does not provide a safe and comfortable environment and although the allegation may have happened or is valid, there is not enough preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is Unsubstantiated.
During Record Review LPA Corral discovered the September 2021 Sign in/Sign out sheets for Classroom # 3 are not completed properly, the time of day for pick up and/or drop off were not included and parent’s or authorized representatives did not sign or include their full legal signature 36 times throughout the Month of September. The following citation is being cited during today’s inspection 101229.1 (a)(1) Sign In and Sign Out states, in addition to the sign-in procedure requirement of Section 101226.1(b), the licensee shall develop, maintain, and implement a written procedure to sign the child in/out of the childcare center that shall, at a minimum, include the following: The person who signs the child in/out shall use his/her full legal signature and shall record the time of day.
LPA Corral conducted an Exit Interview with Assistant Director Evelyn Rodriguez. The report was read reviewed and discussed with Assistant Director. Appeal Rights were explained, Assistant Director was provided a copy of Appeal Rights and signature on this form acknowledges receipt of these rights. Assistant Director was informed all appeals must be in writing and received by the Regional Office within 15 business days. Notice of Site Visit was provided and posted. Assistant Director was informed the Notice of Site Visit must be posted for 30 consecutive days and failure to post will result in a Civil Penalty of $100.00.
