During the staff interview, staff #1 (S1) and S2 confirmed that on Monday 10/18/2021 S2 forgot to pick up Child #1 (C1) & 2 due to schedules changed during the Parent Teacher Conference (PTC) week. The PTC started on 10/18/2021 and the children left the elementary at 12:30pm instead of 2:15pm. On Monday 10/18/2021, the elementary school called the facility at 12:45pm asking facility to come picking up C1 and C2 up since it was 15 minutes late. S2 came immediately to pick the children up.
S1 disclosed that picking up and helping the children with homework are extra services the facility offers to the parents without additional cost and are not listed in the contract.
LPA Nguyen reviewed the children’s files and facility admission policies. The admission policies did not include the transportation provided.
Based on the interviews conducted, the preponderance of evidence standard has been met, therefore the above allegation Staff did not pick up the daycare child from school is found to be SUBSTANTIATED. California Code of Regulations, Title 22, Division 12 & Chapter 1, 101218(a)(6) Admission Policies is being cited on the attached LIC9099D.
Exit interview was conducted. The Notice of Site Visit was posted. Facility representative was informed that the Notice of Site Visit must be posted for 30 consecutive days. Failure to post will result in civil penalty of $100. “The licensee was provided a copy of their appeal rights (LIC 9058 12/15) and their signature on this form acknowledges receipt of these rights.” This report LIC 9099 was provided to the director. First level appeal is to Regional Manager, address is above on the report.
