On 1/21/25, Licensing Program Analyst (LPA) Vivian Trinh and Licensing Program Manager (LPM) Nguyen Tran conducted an unannounced Case Management inspection. LPAs met with Director Tran, Anh Tram and informed the Director purpose of the visit. A tour of the facility was conducted and census was taken. LPA observed 12 preschool children and 2 staff members in St. Michael classroom, 11 preschool children and 1 staff member in St. Raphael classroom, and 7 preschool children and 1 staff member in St Gabriel classroom. Facility was observed operating within compliance with staff and child ratio.

A review of the Facility Personnel Report Summary on this date 01/21/2025 indicates all facility residents, staff, or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.



During the inspection, LPA conducted observation on facility ground and verified alarm devices on front and back gate were functional. Both front and back gates have an additional latch installed. LPAs also verified that the facility outside gates were closed during the visit. LPAs obtained pictures of gates and alarm devices.

During the staff interview, Director stated facility has Garden Grove City’s permit regarding safety devices that were installed on the inside gates of the facility. Director provided a copy of the permit and communication document with Garden Grove City from 05/15/2024 to 09/25/2024 regarding the permit. Director showed proof of scheduled timer for the outside gates to be closed from 10:00 am- 4:00 pm. LPA obtained pictures of the scheduling program myQ application on the Director’s phone. Director stated staff were tasked with checking the gates daily and provided to LPA the copies of the daily Gate Closing and Opening Schedule, and Gate Checklist from 05/15/2024 to 01/21/2025.

During the record review, LPA review file of Staff #1 (S1) who was hired on 05/22/2024 with specific tasks including managing children attendance, particularly during outdoor time in the playground, and ensuring safety gates are maintained throughout the day. LPA reviewed training's, and weekly meeting records for all facility staff members. The records revealed all five (5) current members have been attending online training modules provided by California Early Childhood Online (CECO) and weekly meetings/training's on various topics provided by Director, from 05/15/2024 to present.

There were no Title 22 deficiencies cited during today's inspection.

An exit interview was conducted with Director Tran, Anh Tram port reviewed and discussed. Notice of Site Visit was posted during the visit. The Director was informed that the notice of site visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100. Appeal rights provided and explained. The facility representative was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights.
