25 | Licensing Program Analysts (LPA) Nguyen Tran and Dianna Valdez Santana conducted a case management inspection to follow up on a self-report unusual incident that was submitted to CCL Regional Office on 06/24/2022. It was reported by Miriam Salcedo, Director. According to Director Miriam, on 06/21/22, Adult 1 (A1), reported to the the Assistant Director Sara Sayago, that Child #1 had told A1 about Child #2 (C2) touching C1's private parts in the classroom and in the playground on 06/14/2022. 06/22/2022, Director Salcedo held a conference with A1 and Assistant Director and C1's teacher to address the concern. During the conference, A1 shared about C1's history of sexual abuse and C1 was receiving therapy, however C1 had stopped attending therapy sessions due to the therapist's relocation. Director also brought C1 to the conference and questioned C1 about the allegation, however C1 gave inconsistent information about where C1 was touched by C2. Also, C1 said C2 "just touched me today". Director and teacher reported that C1 and C2 were sitting away from each other in class. Then C2 was brought into the conference for question, C2 was confused and did not know what was going on. According to the Director and teacher, C1 and C2 did not have interactions with each other. Conference ended with a solution that C1 would be moved to another classroom. On 06/23/22, Santa Ana PD arrived to conduct an investigation with the C1 and C2. According to Director, the Police officer left and said they did not find anything in this case. Later on that evening, Director had a conference with A1 and decided to discontinue C1's enrollment pending the investigation.
During today's inspection a tour of the facility was conducted. There were 17 children in 4 classrooms with 5 staffs. A review of staff records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances.
During today's inspection staff members and children were interviewed.
LPA reviewed children and staff records and interviewed staffs and children. LPA also obtained facility's staff's and children roster, notes from teacher-parent conference. Due to insufficient information available at this time and further interviews and document needed, the case manage for the incident needs further investigation.
Exit interview was conducted. The Notice of Site Visit was posted. Director was advised the Notice of Site Visit must be posted for 30 days or 100$ Civil Penalty will be assessed. (End of Report) |
