25 | On 08/04/2022 Licensing Program Analyst, Stella Gutierrez made an unannounced plan of corrections visit to the facility. LPA was met by Karyn Gharib, Business Manager who was explained the reason for today's visit. LPA observed 10 preschool staff caring for 47 preschool children and 17 toddlers.
A review of the Facility Personnel Report Summary conducted on 08/04/2022 indicates all facility staff or other individuals who required caregiver background checks have received criminal record and child abuse index clearances or exemptions.
The facility representative was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
On 07/26/2022 LPA, Gutieerez issued a Type A citation for a lack of supervision. Director was advised to conduct a meeting on the importance of head counts and effective supervision at the facility. On 08/01/2022 the director, Donna Koontz sent an agenda of a meeting conducted on 07/29/2022 that entailed, Safety and Supervision of children and headcount protocols when transitioning children from one area to another. It was also mentioned in meeting agenda and the action plan moving forward of the importance of making sure all doors from unoccupied classrooms are kept shut.
Type A deficiency cleared during today's visit. No deficiencies issued today. A copy of this report and notice of site visit provided to Karyn Gharib, business manager.
