25 | Licensing Program Analyst (LPA) Tran and LPA Trinh conducted a case management inspection to follow up with the additional information received by Centralized Complaint and Information Bureau regarding the complaint control # 06-CC-20230929133517 with allegations (1) Staff member pushed a daycare child and (2) Staff member yells at daycare children. LPAs met with Director Krista Bartolome at the facility and informed the Director the purpose of the visit.
According to the received additional information, Reporting Party (RP) identified 2 children, Child #1 (C1) and Child #2 (C2), whom were alleged being yelled at and pushed by the staff member.
During today's inspection a tour of the facility was conducted. There were 66 children in 5 classrooms with 10 staff members. A review of staff records on 11/30/23 indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances.
During today's inspection, LPA interview C1. C1 did not qualify for the interview. C2 was absent during today's visit. LPA Tran contacted C2's parent, Parent#1(P1) and interviewed P1. P1 disclosed that P1 had heard rumor about alleged allegations and had questioned C2, however, C2 has never confirmed that any staff member at the facility, has ever yelled at or pushed C2.
Based on LPA’s interviews conducted with 1 child and 1 parent, there is no evidence to support any violation of Title 22 regulations. No Title 22 deficiencies cited during today's inspection.
Appeal Rights were explained. The Director was provided a copy of appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. First level appeals should be sent to the regional manager to the address listed above. A Notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Director Krista Bartolome. |
