LPA conducted a visit on 04/23/2024 and interviewed five (5) staff regarding 1) ratio. During the interview with staff, 5 out of 5 staff stated they are never left alone to care for children. 5 out of 5 staff denied their classrooms are ever out of ratio. 3 out of 5 staff stated that is always two (2) qualified staff in each classroom. 3 out of 5 staff stated they follow facility protocol and always request coverage if they need to step out.
LPA reviewed facility attendance sheets for the week of 08/07/2023-08/11/2023 for the Camellia Room. During this period, there were 21 children enrolled. Attendance sheets include the time-of-day children were signed in and out. On 08/07, there were 15 children total. On 08/08, there were 16 children total. On 08/09, there were 15 children total. On 08/10, there were 18 children total. On 08/11, there were 15 children total. LPA also reviewed staff attendance for the Camellia Room that determined there was 2 teachers on 08/07, 08/08, 08/09, and 08/11, On 08/10, there was 1 teacher and an assistant. Throughout the day, during breaks and lunch two teachers covered for those two staff.

During the interview with staff, 5 out of 5 staff stated lead teachers in classrooms are fully qualified to licensing standards. 5 out of 5 staff stated assistants are not left alone with the children at any time. 5 out of 5 staff confirm working with S1 but do not recall how many educational credits they had obtained. LPA reviewed S1’s personnel file and determined that S1 had 9 units that included infant/toddler class.
LPA interviewed six (6) children and there were no disclosures made to LPA.
LPA interviewed three (3) out of 5 parents. Parents did not disclose any information pertinent to the allegations.
Based on LPA’s interviews and record review, 1) facility is operating out of ratio and 2) unqualified staff are caring for children may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated.
Exit interview was conducted with Director Krista Bartolome. The Notice of Site Visit was posted. Facility representative was informed that the Notice of Site Visit must be posted for 30 consecutive days. Failure to post will result in civil penalty of $100. Licensee was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. (End of Report)
