25 | Licensing Program Analyst (LPA) Dean Thompson conducted a case management investigation to follow up on a self-report unusual incident that was submitted to CCL Regional Licensing Office on 12/5/2023. Upon arrival LPA was met with Director Ana Rivas. There were 22 infant children with 10 staff members during todays visit.
During the inspection it was determined the facility is operating within its licensed capacity and within compliance of staffing ratios. A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.
On 12/5/2023, a self-reported Unusual Incident Report (UIR) was filed with the Licensing Office. The facility reported on 12/1/2023, child#1 (C1) fell inside the Rosemary classroom. On Sunday, 12/3/2023, the director received an email from the parents stating on Saturday, 12/2/2023 C1 was not barring weight on the left leg. On Saturday 12/2/2023 the parent stated C1 would cry during diaper changes. The parent took C1 to urgent care Saturday, 12/2/2023 and X-rays showed C1 had a fracture on the left leg.
Based on interviews which were conducted, and records reviewed, there is no evidence to support any violation of Title 22 regulations. No Title 22 deficiencies are being cited during today's inspection.
Exit interview conducted and report was reviewed with Director Ana Rivas. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
Appeal Rights discussed. Director was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. |
