32 | Reporting Party (RP) stated a child eloped from the facility without staff supervision.
On 05/07/24 co-complainant stated Child 1 (C1) was found by child representative outside of the facility without supervision.
On 5/8/24, LPA interviewed two (2) staff. Staff 1 (S1) stated S1 was with another staff 2 (S2) while the class was in the outdoor play area, S1 stated while both S1 and S2 were administering first aid to another child, S1 stated S1 was notified by a parent that a child went out of the front entrance door. S1 went to the door and saw C1 already outside in the stroller with C1’s sibling. S2 stated the class was in the outdoor play area and while S2 and S1 were administering first aid to another child, S2 did not witness C1 ran away.
On 5/30/24 S1 stated the original door latch bolt does not work that is why when people coming in or going out do not latch the second deadbolt, the door will not shut. S1 stated that the cameras around the facility does not record but only has live view.
On 5/30/24, LPA observed the door will not shut when the second bolt (Electronic Keypad Deadbolt) is not turned to lock and the latch bolt from the original door knob is facing the wrong way and not locking, that is why the door won't shut on its own even when pulled to close.
On 05/20/24 LPA interviewed four (4) parents, none of the parents interviewed disclose any information that could support the allegation.
On 05/28/24 LPA received a copy of Call Detail Information Report dated 05/02/24 from OC Sheriff’s Department regarding the incident that took place in the facility. The report stated no crime observed.
Based on observation, interviews conducted, and records reviewed, it has been determined that a child eloped from the facility without staff supervision when the child representative saw C1 outside at the front area of the facility. The preponderance of evidence standard has been met; therefore, the above allegation is found to be Substantiated.
