25 | On 05/23/24 Licensing Program Analyst (LPA) Anna Chan conducted a case management deficiency report. LPA met with Director Amanda Gonzalez and informed the Director of the purpose of the visit. During today's visit, a tour of the facility was conducted. The overall census observed was 21 preschool children and 5 staff.
During a complaint investigation on 04/22/24, while LPA was touring the outdoor activity space, LPA discovered that the preschool outdoor space had 2 parked cars inside. LPA observed there is a small rack barricade less than 4ft in height going to the area where the cars were parked. Based on the facility sketch, it is not indicated that the fenced lot includes parking, but instead it only indicated outdoor play area.
On 4/22/24, during staff interview, Director stated that there is always a teacher standing in between where the cars are parked and the outdoor play area.
Director was provided with a copy of Plan of Operation Regulation 101173 by LPA on 5/23/24.
Based on observation and interview, the following deficiency was discussed and cited. The facility was not in compliance with the California Code of Regulations, Title 22, Division 12, Section 101173(b)(7)Plan of Operation. Please refer to LIC809D for details of deficiency.
Exit interview was conducted. Report and deficiency reviewed and discussed. Notice of Site Visit was posted during the visit. Director, Amanda Gonzalez was informed that the notice of site visit must be posted for 30 consecutive days. Failure to post will result in civil penalties of $100. Appeal rights provided and explained. The Licensee was provided a copy of their appeal rights (LIC 9058 01/16) and their signature on this form acknowledges receipt of these rights. First level appeals should be sent to the regional manager to the address listed above.
