LPA Nguyen conducted a telephone interview with six staff members and was able to talk to four staffs. No disclosures were made by staffs to LPA Nguyen. All interviewed staff members denied of not preventing day care children from being bit by another child in care and not preventing day care children from being scratched by another child in care. No staff member disclosed witnessing any staff not preventing day care children from being bit by another child in care and not preventing day care children from being scratched by another child in care.
LPA Nguyen conducted interview to 3 children. No disclosures were made by children to LPA Nguyen. All interviewed children disclosed they like going to the school.
LPA Nguyen contacted eight parents by phone and was able to interview four parents. All four interviewed parents stated they like the facility and all staff members are very nice. Interviewed parents also stated they did not have any concern or issue with the facility. LPA Nguyen attempted to interview subject child but not available for interview.
Based on the information gathered from LPAs' interviews, there is insufficient evidence to corroborate the allegations. Although the allegation may have happened or are valid, there is not a preponderance of evidence to prove the alleged Staff not preventing day care children from being bit and scratched by another child in care did or did not occur, therefore both allegations are UNSUBSTANTIATED.
Exit interview was conducted. The Notice of Site Visit was not posted due to tele-investigation Covide-19 State of Emergency. Appeal Rights was explained. A copy of appeal rights (LIC 9058 1/16) will be provided through email and their signatures on this form acknowledges receipt of these rights. First level appeal is to Regional Manager, address is above on the report.
