Licensing Program Analysts (LPAs) D. Valdez Santana and A. Nunez conducted an on-site inspection for the purpose of a Case Management Incident Inspection on 1/22/2025. LPAs toured the facility inside and outside, census was taken in individual classrooms, (Coyotes had 17 children, 4 staff, Tortoises had 16 children, 5 staff and, Bears 21, 4 staff) there were 54 children present upon arrival and 13 staff. Today's Case Management inspection is being conducted in response to a self-reported incident, reported to the Regional Office on 12/18/24, detailing a child #1 (C1), had been left in and locked inside a bathroom by staff #1 and found minutes later by Staff #2 (S2).

During the inspection, LPAs interviewed three staff, child #1 (C1) involved in the incident was not interviewed due to not being present. LPAs also inspected the bathroom C1 was left in. LPAs reviewed the Incident Report with facility representative as well as the response of the facility. On 12/18/2024, Staff #1 (S1) disclosed taking children to the bathroom including C1, saw the 3 children leave the restroom and forgot that C1 was still in the restroom. S1 locked the door and went to the long table that is approximately 10 feet away from the restroom. Staff #2 (S2) returned to the bathroom area and heard crying from the restroom. S2 asked S1 if someone was in the restroom, initially S1 said no and then remembered C1 was still in there. S1 immediately opened the restroom door, S1 and S2 found C1 crying by the door. S1 and S2 estimated that C1 was left and locked in the bathroom for about 4 minutes. S2 disclosed when returning from the rest and refresh area with a child in hand, that when returning to zone 1, S2 heard crying from the restroom. S2 asked S1 if anyone was in there, S1 said no, but heard the crying again and then S1 remembered C1 was in the restroom. From the information LPAs gathered during this inspection and staff interviews, LPAs determined that the facility staff failed to provide adequate supervision and C1 was left and locked in the bathroom alone for about 4 minutes, S1 did not even realize C1 was gone, until S2 heard C1 crying from the bathroom this is Title 22 violation.

Therefore, in the areas that were inspected, one Type A deficiency was observed of the California Code of Regulations, Title 22, Division 12 Section 101229(a)(1) Responsibility for Providing Care and Supervision. See attached LIC809D

LPA Valdez Santana informed facility representative, that this report dated 1/22/2025 documents one Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care.

LPA Valdez Santana also informed the facility representative to provide a copy of this licensing report dated 1/22/2025, that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

Exit interview was conducted. The Notice of Site Visit was posted for no less than 30 consecutive days. Appeal Rights was explained. A copy of appeal rights (LIC 9058 1/16) was provided and their signatures on this form acknowledges receipt of these rights. First level appeal is to Regional Manager, address is above on the report.

evidenced by: Based staff interviews, S1 failed to ensure to provide supervision at all times. C1 was locked in an outdoor restroom alone for 4 minutes without any supervision. This poses an immediate Health and Safety risk to the children in care.
