25 | Licensing Program Analyst (LPA) P Rivas conducted a case management visit to correct information in reference to the outside activity space on a report issued on 12/07/23. In all other aspects, the report issued on 12/07/23 remains the same.
The report should have indicated that the outdoor activity space measured at 1170 square feet, which is sufficient to accommodate the requested capacity.
LPA took census upon entrance and noted 8 school age children and two teachers.
A review of staff files indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.
LPA viewed sufficient equipment and supplies which include but not limited to toys, chairs tables, manipulatives, art supplies, books,, speakers that were age appropriate for children in care.
LPA advises Ms. Rhee (via telephone) that a waiver needs to be submitted regarding school age children and pre school children sharing the outdoor activity space. Waiver to include facility sketch Lic 999 including inside and outside, schedule of use.
Also needed is Director's copy of Administration class, updated lic 500 with hours to match lic 200a, and second permission site with letter . New lic 200a(03/23).
Applicant will provide items by 01/12/24.
A notice of site visit was given to Ms. Jun and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days
Exit interview was conducted and report was reviewed with Director, Ms. Jun. |
