In the areas that were evaluated, the following 10 deficiencies were observed of the California Code of Regulations, Title 22, Division 12 or Health & Safety Codes at the time of the visit and are on the attached LIC809D. 1596.954 Licensure Requirements (non-working carbon monoxide detector), 101439.1(f) Infant Care Center Sleeping Equipment (blanket inside crib), 1597.16(a)(1) Lead Testing, 101170(e)(2) Criminal Record Clearance (staff not associated to infant license) 1596.7995(a)(1) General Provisions and Definitions (1 staff missing immunization record and health screening), 101419.3(a) Modifications to Infant Needs and Services Plan (individual plans not updated quarterly), 101220(b)(1) Child's Medical Assessments (missing immunization records), 101212(b) Reporting Requirements (missing directors packet), 101427(h) Infant Care Food Services (infant not in high chair while eating).
Exit interview conducted and report was reviewed with the Interim Director Christina Carr. A notice of site visit was given and must remain posted for 30 days.
Appeal Rights and deficiencies were discussed. The facility representative was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days.
