Licensing Program Analyst (LPA) Mahnaz (Nancy) Malek conducted a case management inspection of the facility for the purpose of combing two rooms of 112A and 112B by removing the wall in between and converting the two rooms to one large room (112). The facility has been licensed for 48 children in Rooms 107, 112A and 112B. The facility is not changing capacity. LPA met with staff, Maria Wiese who gave a tour of the facility to LPA. During inspection, PA met with Early Learning Specialists, Aldo Aceves and Veronica Berber. The facility is located on the John Muir Fundamental Elementary School Site. The director Claudia Reeves is in transitioning and the new director is Yolanda Gutieres Lilly who was present today in room 107.

The facility is running Title 5 program and operating from 8.00 am to 4.00 PM, Monday to Friday serving children ages 2-5 years old offering two half day sessions in Room # 112 and Full day session in Room # 107. Facility has the approval letter from the School Superintendent giving permission to use the school premises. There were 8 children with 4 staff present today.

A review of the Facility Personnel Report Summary on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. (The staff are employees of Santa Ana Unified School District.

Room 107 offers morning and afternoon sessions. Morning session is from 8:00am to 11:00 am and afternoon session is from 1:00 pm to 4:00 pm. The facility provides morning breakfast and afternoon lunch to children. Children do not nap in this room. Room 112 is full time program from 8:00 am to 4:00 pm. Children nap in this room. The children are dropped off and are singed in and out at the entrance electronically. The facility has met all the posting requirements.

All areas identified on the Facility Sketch were inspected. If children become ill during the day, they will rest in the isolation area located, in the nurse’s office, next to the school office, away from the other children, and use the adult restroom.
Following was observed in room 107 and 112. Some equipment for children were secured during inspection.
Age appropriate furniture and activities including Educational / Arts & Crafts items.
Room is well ventilated with proper lighting and shade.
There is a working Smoke detector / Carbon Monoxide detectors and Fire Extinguisher.
Classroom # 112 has two sinks and two toilets inside the room, while room # 107 uses the bathrooms located outside room # 107 on the campus. There is a waiver on file for this purpose..
During full day session children nap on the cots, linens and blankets are provided by the facility and washed on a weekly basis by the parents.
Outdoor Activity Space: Playground is completely enclosed. Outdoor activity area is supplied with age and size appropriate equipment with adequate of cushioning materials under the climbing structure. Shade is provided by Canopies and building shade. The play yard is shared with the Kindergarten and children of Special Education Program. There is a waiver to share the playground on file.
Fire clearance approval has been received approving the requested capacity of 48 children in rooms 107 and 112.
The measurements of two rooms are as below:
Room 107 is 964.10 square feet
Room 112 is 1218.15 square feet
Total indoor measurement for rooms 107 and 112 is 2182.25 square feet divided by 35 square feet = 62.35 (62 children).
Toilets: 6 X 15= 90 children
Urinal: 1 X 5= 5 children
Sinks: 7 X 15 = 105 children.
There are 3 toilets, 2 sinks in the Girls bathroom and one toilet, one stall , and 2 sinks in the Boys bathroom. There is one sink in room 107. There are two toilets and 2 sinks inside room 112. The facility has a waiver to share restrooms on the Elementary Campus.

The classroom is equipped with carbon monoxide detector. The new Site Supervisor is Yolanda Gutierez Lilly. She is in transitioning to this site. The director's packet for her was submitted to LPA during inspection. The staff file were checked and they are all current on CPR and 1st aid, H &S and Mandated Reporter Training.

Orange county Fire authority has approved the requested capacity of 48 preschool children in rooms 107 and 112.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platform.

A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Exit interview conducted and report was reviewed with the Site Representative, Veronica Berber.

Based on today’s measurements, the sinks and toilets availability, the request of the facility for converting rooms 112A and 112B to one large room a capacity of 48 children in rooms 107 and 112 will be final reviewed and discussed with management in the Regional Office. The facility will be notified if any additional information is required.
