On 12/5/24 Licensing Program Analyst (LPA) Nunez conducted an unannounced Case Management visit to ensure that the facility is staying within compliance of Title 22 regulations.

LPA met with Director Angelica Burgos. LPA took census in the infant and toddler option classrooms. There were 4 infants with 1 staff member in room number 2 and no toddlers in the option program which is in room number 3. The facility was operating within its licensed capacity and within compliance of staff-to-child ratios. An on-site Facility Personnel Report Summary review showed that all facility residents, staff, or other individuals who require background checks have received criminal record and child abuse index clearances or exemptions.



During the inspection in room 2, LPA observed 4 infants playing with materials in the classroom. Staff files were reviewed for staff present during the facility inspection on this date, 2 staff files were reviewed. The 2 staff members that were in the infant classroom were fully qualified with infant/toddler units. Children's records were reviewed, and there was a separate, complete and current record for each child. In the areas reviewed the children’s files were found to be in full compliance.

In the areas that were evaluated, no deficiencies were observed of the California Code of Regulations, Title 22, Division 12 at the time of the visit.

An exit interview was conducted with Angelica Borgos . Appeal Rights were explained. The Director was provided a copy of appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. First level appeals should be sent to the Regional Manager to the address listed.
The facility representative was informed that the “Notice of Site Visit” must be posted for 30 consecutive days. The “Notice of Site Visit” must be posted on or adjacent to the door. Failure to post will result in Civil Penalties of $100.00.
