1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	Page 1 of 2 Licensing Program Analyst (LPA) Mahnaz (Nancy) Malek conducted a case management inspection of the facility for the purpose of the facility has remodeled the preschool playground with some changes. The facility has been licensed for 132 preschool children age 2 to 5 years old in rooms 117, 118, 119, 120, 121, 122, 123, and 124. LPA first met with Associate Head of School, Lexi Powell. The classrooms were inspected. There were no children present due to Thanksgiving Week. The director, Juanita Espinoza arrived during inspection. LPA inspected the new playground. The measurement of the playground according to pre- licensing inspection date of 4/28/21 is: 10009 square feet divided by 75 = 133.46 ( 133 children). There is no changes in measurements. LPA observed there were appropriate fencing around the playground. The facility had added screening on the fences. There are adequate new flooring (Turf) is around and under falling zones of the play structure with slides. The equipment were inspected and they are all age appropriate such as sensory equipment, see-saw and climbing equipment. There are two exist gates on this playground which are child proof locked. LPA reminded the director, Juanita Espinoza to make sure those gates are closed and locked when children present. One exit leads to the outdoor space for Private Middle School and another exit gate leads to Private School outdoor space. No hazardous observed. The playground is ready to be used for children. A review of the Facility Personnel Report Summary on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

The director, Juanita Espinoza was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
