Licensing Program Analyst (LPA) Romy Castanon conducted a case management inspection to follow up on reported incident information submitted on 04/22/2024. LPA met with Director Kari Kimbrough and explained the reason for the visit. LPA conducted census in five (5) individual classroom and two (2) classrooms in the preschool playground. There were a total of 159 children and 16 staff members.

A review of staff records on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances. LPA assisted Director in associating one staff member to their preschool license.

Incident information stated there has been food service issues since the facility chef has left that included an allergic reaction to a child. During today's visit, LPA interviewed four (4) staff members. All four (4) staff members explained their roles during food service times. All staff denied observing any form of food service issues. All staff denied any knowledge of recent allergic reactions of children in care. All staff were able to explain the facility’s allergy and food restriction policies, as well as, how they prevent any allergy related issues in the classroom.

During interview with Director, LPA was informed that a self-reported incident due to an allergic reaction was cited by the Licensing Department on 09/12/2023. Director stated there have not been any recent food service issues or allergic reactions. Director is in the process of hiring qualified kitchen staff. Until the facility hires a new chef, the Director and Assistant Director will continue to prepare food for daycare children. Director will continue to communicate and assign appropriate staff when assistance is needed in the kitchen or with food deliveries.

Based on LPA’s interviews conducted, there is no evidence to support any violation of Title 22 regulations. No Title 22 deficiencies cited during today's inspection. Exit interview was conducted with Director Kari Kimbrough. Notice of Site Visit was posted during the inspection. Facility representative was informed that the notice of site visit must be posted for 30 consecutive days. Director was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. (End of Report)
