25 | Licensing Program Analyst (LPA) Dean Thompson conducted a case management investigation to follow up on a self-report unusual incident that was submitted to CCL Regional Licensing Office on 3/4/2025. Upon arrival LPA met with Director Delmy Cuevas then toured the facility. Census was taken, there were 20 preschool-age children with seven staff.
During the inspection it was determined the facility is operating within its licensed capacity and within compliance of staffing ratios. A review of the Facility Personnel Report Summary on this date indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.
On 3/4/2025, a self reported Unusual Incident Report (UIR) was filed with the Licensing Office. The facility reported on 3/4/2025, child#1 (C1) falling on side of the play structure bottom stairs resulting in hitting the part under C1 bottom lip. C1 didn't require any medical attention.
No deficiencies were observed of the California Code of Regulations, Title 22, Division 12 at the time of the visit.
Exit interview conducted and report was reviewed with Director Delmy Cuevas. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
Appeal Rights discussed. Director was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. |
