During the investigation, LPA conducted unannounced facility visits on 06/17/2026 and 06/24/2026; obtained the Personnel Roster, Children's Roster (Child Directory), staff work-week schedule dated June 8, 2026 – June 12, 2026, and sign-in/out sheets for four staff dated June 11, 2026; reviewed seven staff files; conducted eight staff interviews (S1–S8); conducted 2 parent interviews and did not interview infants due to limited vocabulary.
During the unannounced visits on 06/17/2026 and 06/24/2026, LPA toured all indoor and outdoor areas accessible to children. LPA observed 2 (two) two qualified staff supporting 6 (six) infants with various needs during nap and feeding times.
During LPA staff interviews, 8 out of 8 staff (S1–S8) stated they have never witnessed an unqualified staff member being left alone with children. Staff explained that to be considered a qualified teacher, individuals must have Early Childhood Education units, immunization's, Child Mandated Reporter Training certification, and completed background checks. 3 (three) staff reported that non-classroom personnel have occasionally assisted briefly while paired with a qualified staff member until a substitute arrived. This practice is permitted under Title 22, Division 12, Chapter 1, Article 6, Section 101216.3 (Teacher-Child Ratio). Subsection (h) states that persons employed for clerical, housekeeping, or maintenance duties shall not be counted as teachers in the teacher-child ratio; however, Subsection (1) permits the licensee to use such persons as emergency substitutes while a qualified teacher substitute is being secured. Interviewed staff stated that they use in-house staff or sub-contracted services to support the classroom and have not had any concerns regarding unqualified staff, nor have they heard of any such concerns.
During the records review, LPA reviewed 7 (seven) staff files and confirmed all staff met the qualifications required to work with children. LPA also reviewed the staff work-week schedule dated June 8, 2026 to June 12, 2026, which showed 14 staff scheduled to work between 6:55 AM - 6:00 PM. Additionally, sign-in/out sheets for 4 (four) infant staff dated June 11, 2026 documented that each staff worked eight hours.
