32 | In addition, per CCC 101217 Personnel Records. (a) The licensee shall ensure that personnel records are maintained on the licensee, administrator, and each employee. During a staff file review, the LPA observed S3, S4, and S5 did not have all the required documentation in file; among the missing documents was a health screening, termination date for S4 and S5, and a tuberculosis test for S3 and S4. A Type B was issued.
ROSTER: The LPA observed C1 was not included in the roster. The licensee stated the child was dropped off at school on 11/14/24, which means the child was in attendance in the childcare center before being taken to school. Per HSC 1596.841 Current roster of children provided care in facility required; Each child day care facility shall maintain a current roster of children who are provided care in the facility. This was corrected during the visit. A technical violation was issued.
The facility was not in compliance. Violations of the California Code of Regulations, Title 22, Division 6 were observed, discussed, and cited at the time of the visit. The following violations were cited: CCr 1101221 Child's Records(a), 1596.7995(a)(1) Employees or volunteers at day care center; immunization requirements; records; exemptions, 1596.841 Current roster of children provided care in facility required and 101217(a) Personnel Records.
The LPA conducted an exit interview and reviewed the report with the licensee. The Notice of Site Visit was posted. The licensee understands that the Notice of Site Visit shall remain posted for 30 days. The Appeal Rights were explained. The licensee received a copy of the Appeal Rights (LIC 9058 01/16), their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. First-level appeals should be sent to the Regional Manager at the address listed above.
