Licensing Program Analyst (LPA) Ms. Ketki Desai conducted an announced on site pre-licensing inspection at the childcare center. Facility is now adding the Infant License. LPA met with the Administrator Ms. Eliezrie, Naomi who gave a tour of the new Infant classroom on the premises of North County Chabad Center Church. One Infant room (B) and outdoor area has been designated for Infant care. Initially room B was under preschool license. Now facility has one classroom for each of the Program. Room A: Preschool and Room B is Infants.

The applicant has requested to provide care and supervision for 15 infants (3 months to 2 years ) Monday through Friday 8:30 AM. to 3.30 PM. in the assigned one Infant classroom.

LPA observed staff following CDC and Dept of Public Health Guidelines. Due to COVID Pandemic parents shall be encouraged to follow the CDC and COVID requirements.

A review of the Facility Personnel Report Summary on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.

The facility is on the premises of North County Chabad Center, with parking area for parents. Manual sign in table has been set up at the entrance of the infant room, where parents shall sign in and the children shall enter through the designated classroom door. Upon arrival each child has an health evaluation along with hand washing before they participate in the activities.

Isolation area is the Director’s office, located at the entrance of the facility and there is an additional cot with linens for the sick child in the Director's office.

Safe sleep posters / SIDS

The following was observed in the Infant room
· Age appropriate furniture and equipment’s
· Toys, tactile equipment's
· Room is well lighted with ventilation.
· Infant room has a napping area with cribs for younger infants and cots for older infants
· All Infants needed items are brought from home.
There is sufficient open area for the child to move
Changing table is placed at a arm's length of the sink,there are two sinks in the classroom.
Storage cubbies are arranged to store personal belonging
Napping area: Currently there is fencing placed around the napping area, and applicant is waiting for the new wooden fence to be delivered. Napping area shall have four cribs.
Cribs: Have a mattress with tight fitting sheets.
One crib shall be used by one infant at a time only.
Flooring: Vinyl flooring with play pads and non-slip rugs in the area.
All Infant items shall be provided by the parents : Diapers/ Wipes/ Lotions / formula and Mother's milk
Facility does have additional items if needed.
There is a Refrigerator in the classroom to store the milk and other items. (2)

LPA discussed the Safe sleep practice with the Applicant Eliezrie, Naomi

· Always place infants on their backs for sleeping
· Use only a tight-fitting sheet on the crib or play yard mattress
· Do not hang any items from the crib or above the crib
· Keep all items, including blankets, out of the crib or play yard
· Pacifiers may be used as long as they do not have items attached to them
· Infants should not be swaddled or have any items covering them while sleeping
· The temperature of the room should be comfortable enough for an adult to wear a T-shirt and not be too hot or too cold.

LPA measured the assigned Room B: Indoor activity space : 487'21 divided by 35 = 13'92 ( 14 infants)
Facility can accommodate only 14 infants in the assigned room. Not 15 infants as requested.

Total sinks : 2 X 15= 30 children : Infant room has 2 Potty chairs if needed for older infants.

Infant outdoor play yard : Facility has designated Infant yard, besides the preschool yard, with a tall fence in-between and following was observed:
· Playground is fully enclosed by an appropriate fencing
· Outdoor activity space is supplied with age and size appropriate equipment, outdoor activity toys,
· An adequate amount of cushioning material: soft padded carpets
· Adequate shade is provided by shaded pull up ease ups
· There is also a bike trail area and a cemented open area for Infants to play.
Strollers shall be used from the classroom to the yard, which is across the room.
Facility does need to add additional age appropriate activities and structures for infants and clean the side bushes in the infant yard. There is an Emergency exit door with a lock installed and key remains the Administrative staff.
Outdoor activity space: 1531'39 divided by 75 = 20'42 (20 Infants )

Facility has sufficient space to accommodate 14 Infants in the designated Infant yard.

Fire clearance received from Orange County Fire Authority approving the requested capacity of 37 children in the two rooms( Room # A Preschool and Infant room B).

Applicant Ms. Eleizrie, Naomi is current on the required Immunization/ Pediatric CPR/ First training valid through May 1st 2023 and have provided the completion certificate for the Preventive Health training (Nutrition and Lead component and Mandated Child abuse reporting training.

Following items were discussed at the time of Exit interview.
Criminal Record Statement : Applicant, Ms. Eliezrie, Naomi was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
Safe Sleep : LPA discussed the safe sleep regulations with applicant Ms. Eliezrie, Naomi discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed applicant, Ms. Eliezrie, Naomi of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Facility needs to complete the following items before a final license can be issued


(1) Replace the wooden fence surrounding napping area
(2) Set up the infant yard with additional activities
(3) Clear side bushes in the Infant play yard.
Applicant shall submit the proof of correction by September 10th 2022 via email to LPA

A license for 14 infants ( 3 months to 2 years ) in Infant Room B
Monday to Friday 8.30 am to 3.30 PM, shall be issued upon receiving the above corrections and Management approval.
