Licensing Program Analyst (LPA) Mahnaz (Nancy) Malek conducted an announced on site pre-licensing inspection at this childcare center for the purpose of operating a new preschool program. LPA met with the Applicant Margaret Colasante and director, Jessica Cisneros who gave a tour of the preschool rooms. Two rooms and outdoor area have been designated for preschool program.

The applicant has requested to provide care and supervision for school age children. The facility has requested for ages 5 to 14 years old in rooms 5 and 6, Monday through Friday from 6:00 am to 6:30pm in the assigned rooms # 5 and 6. This program is for before and after school hours and on the days public schools are closed on a full time basis.

LPA observed staff following CDC and Dept of Public Health Guidelines. Due to COVID Pandemic parents shall be encouraged to follow the CDC and COVID requirements.

A review of the Facility Personnel Report Summary on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.

The sign in/out procedures was reviewed which is set up at the entrance through the gate. Parents sign in and enter the designated rooms. This is a before and after school program. The staff will pick up children from school sing them in and the parents will sign the children out. Upon arrival each child has an health evaluation along with hand washing before they participate in the activities.

Isolation area is the office area that has been set up an area to isolate the sick child. Separate mat and linens are available for the sick child. The nurse office on the campus has additional cot with linens and shall attend the sick child as well.

LPA discussed the posting requirements including, but limited to, the following:
· Facility License in public area (101160)
· Emergency Disaster Plan (LIC 610)
· Earthquake Preparedness Check List (LIC 9148)
· Parents’ Rights Poster (PUB 393)
· Personal Rights (LIC 613A)
· Menus / Activity Schedule
· Notice of Site Visit (LIC 9213) and Type A deficiencies / Plan of Corrections of Type A deficiencies
· Granted Waivers (available for review)
· Child Car Seat Law (PUB 269) & · COVID Posters.


The following were inspected in the indoor activity space: Room # 5 and 6.
· Classrooms are adequately equipped with age and size appropriate furniture and equipment
· Drinking water is available inside through personalized water bottles.
· Sign in/Sign out procedure was reviewed and meets regulation requirements.
· Facility has in-built smoke detectors & carbon monoxide detectors are plugged in the wall with Fire Alarms and fire extinguisher mounted that meet statutory requirements.
· Cubbies available for storage of individual child’s personal belongings
· Facility provides early breakfast AM/PM snacks upon returning from school and during full time staying.
Staff shall walk the children to the restroom area which are located on the court yard.
Rooms are well lighted with proper ventilation
Each room has age appropriate furniture and equipment's for the ages to be served.
There are sinks and toilets inside each classroom with additional toilets and sinks on the school premises. Medications are stored in the locked cabinets. First Aid kit and Emergency kit is present in the classroom.

LPA measured the assigned room for school age rooms 5 and 6:

Indoor activity space for school age rooms 5 and 6 are 1100 square feet divided by 35 = 31.42 which accommodates 31 school age children.
Outdoor activity space: School age yard, Basketball Court, side patio: 8798 square feet divided by 75 = 117.3 which accommodates 117 school age children.
Total sinks : 3 X 15= 45 children
Total toilets: 2 X 15 = 30 children

School age play yard : Facility has sufficient play yards on the premises :

· Playground is fully enclosed by an appropriate fencing


· Drinking water is available outdoors by personal water bottles.
· Outdoor activity space is supplied with age and size appropriate equipment, including climbing play structures and outdoor activity toys.
· An adequate amount of cushioning material such as mulch are under the climbing structures.
· Adequate shade is provided by trees.
· Children also have access to the outdoor patio for doing art work. The patio is connected to room # 5
The play structure is safe for the age group to be served (5 and above)


Based on the indoor /outdoor measurements, sink and toilet availability, facility has sufficient space to accommodate 30 school age children.

Review of records to be maintained

Child Care Centers :LPA reviewed with Applicant Margaret Colasante and director, Jessica Cisneros the LIC 311A, Records To Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.


Fire clearance received from Orange county Fire Authority approving the requested capacity of 30 school age children in the assigned rooms of 5 and 6.

Administrator on site Ms. Cisneros is current on the required Immunization/ Pediatric CPR/ First training and has provided the completion certificate for the Preventive Health training (Nutrition and Lead component and Mandated Child abuse reporting training.

Following items were discussed at the time of Exit interview.
Criminal Record Statement : Applicant Margaret Colasante and director, Jessica Cisneros were reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

A license for 30 School age children (5 -14 years) in rooms 5 and 6, Monday to Friday : 6:00am to 6.30 pm. Per Fire Department's instructions room # 5 not to exceed 27 children and room # 6 shall not exceed 24 children.

Facility needs to complete the following items before a license is issued

(1) Final clean up of the Basketball court (Picture)

(2) A revised sketch (LIC 999) to reflect the infant rooms.

(3) Written Plan for Incidental Medical Services (IMS).


(4) Three months bank statements.
(5) Posting requirements.

Applicant shall submit the proof of correction within 30 days to LPA.
