1 2 3 4 5 6 7 8 9 10 11 12 13 14 15 16 17 18 19 20 21 22 23 24 25	Page 1 of 3 Licensing Program Analyst (LPA) Mahnaz (Nancy) Malek conducted a case management inspection of the facility for the purpose of increasing capacity by adding another room (Room # 1) to the existing Infant/Toddler Program. LPA met with director, Jamie Luu who gave a tour of the school to LPA. Currently the facility is licensed for 6 infants and toddles ages 3 to 24 months in Room # 8. LPA observed there were 2 staff with 5 children in this room. The facility is on the premises of The Liberty Baptist Church and Newport Christian Private School on the same premises. A review of the Facility Personnel Report Summary on this date indicates that all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions. The facility has requested to provide care and supervision to 16 infants and toddlers ages 3 to 18 months old in the new room (Room # 1) , Monday to Friday from 7:30 am to 6:00 pm. The facility runs a full time and part time program. The children are dropped off, signed in and out electronically at the entrance of the building to go to room # 1 The facility has met all the posting requirements. All the classrooms were measured. The indoor activity space measurement for the current requested capacity is as follows: Room # 1 is: 617.56 square feet Napping Room is 219.22 which is not included in the activity measurement. There is a sink in the classroom that there is a changing pad with raised sides (two sides) at least 3 inches (This pad has a raised sides of 7 inches). The vinyl pad is located on the sink counter within the arm's reach of a sink. The director stated there are always additional staff in the infant room to supervise children while the other staff is changing diapers. LPA discussed the procedures of diaper changing with director. She said the staff disinfect the diaper changing surface before and after each use and follow the procedure and wash the child's hands when done. Continued on page 2

The director showed LPA the procedure for diaper changing which is posted. Never leave the child unattended on the diapering pad.
Total outdoor activity space measurement is as follows:
Outdoor space for the infant/toddler playground is: 915.83 divided by 75 = 12.21 (12 children).
Total of potty chairs are available for this age group of 3 months to 18 months old if needed.
Total of sinks: 1 X 15 = 15 children
The facility is equipped with carbon monoxide detector. All the classrooms have age appropriate toys and equipment.
A copy of the 2016 “A Child Care Providers Guild to Safe Sleep” was provided to the applicant/facility representative.

· Always place infants on their backs for sleeping
· Use only a tight-fitting sheet on the crib or play yard mattress
· Do not hang any items from the crib or above the crib
· Keep all items, including blankets, out of the crib or play yard
· Pacifiers may be used as long as they do not have items attached to them
· Infants should not be swaddled or have any items covering them while sleeping
· The temperature of the room should be comfortable enough for an adult to wear a T-shirt and not be too hot or too cold.
The director was reminded of safe sleep regulations and logging every 15 minutes check on infants and toddlers under 24 months old. A sample of the log was given. LPA also discussed the items not allowed in the childcare facilities for infants and toddlers. A flyer of the items was given to director.

Orange county Fire authority has approved the facility for maximum of 16 infants and toddlers in Room # 1 but the minimum factor for the assessment is the number of sinks for this program.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platform. A notice of site visit was given and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Exit interview conducted and report was reviewed with the director, Jamie Luu.

Based on today’s indoor and outdoor measurements, the sinks and potty availability, the request of the facility for moving to a new room (Room # 1) for 15 children ages 3 months to 18 months will be final reviewed and discussed with management in the Regional Office and after receiving the following. The facility will be notified if any additional information is required.

Please submit the following no later than 30 days of today's date.

1- Send a picture of Room # 1 and napping room after moving the furniture from the old room.

2- Updated waiver request for playground for the increased capacity of 15 infants and toddlers while infant/toddler playground measures for 12 children. For waiver, please send request letter, LIC 999, LIC 500, outdoor schedule including transitioning time for each group, and a photo of the playground.
