During the investigation, LPA conducted observations on 05/01/24, 05/22/24, 05/28/24 and 06/05/24, and reviewed camera footage of preschool classrooms and playground on 05/16/24 and on 05/22/2024 and interviewed 7 staff members.
During observations on 05/22/2024 and 06/05/2024, the facility classrooms were observed to be operated out of ratio.
Upon arrival at the facility at approximately 8:15am 05/22/2024, LPA met with the Assistant Director and Administrative staff in the front office and informed facility representatives of the purpose of the visit. As LPA started the tour of the facility. LPA observed Assistant Director walked to the Younger Twos classroom 26 preschool children with Staff #3 and Staff #4 in classroom Older Twos. LPA observed Assistant Director walked to the classroom to assist the staff in classroom Older Twos at approximately 8:30am. Assistant Director transitioned some children from this classroom to another classroom to meet ratio in classroom Older Twos. The Older Twos classroom was out of ratio (2 teachers supervising 26 preschool children) prior to 8:30am when Assistant Director transition the children over to the Younger Twos classroom.
During this visit, at 2pm, LPA observed 1 staff supervising 16 children, in which 13 children were napping and 3 children were awake including 1 child sitting at the table and was reading a book in classroom Threes and 1 staff supervising 10 napping children and 5 children who were awake on the napping equipment.
During record reviews, facility camera footage on 05/22/24 shown classroom Older Twos had 2 staff members supervising 25 preschool children at 8:21am. At 8:23am another child arrived at the classroom Older Twos. Thus, the Older Twos classroom to have 2 teachers supervising 26 children. The classroom was operated out of ratio until Assistant Director came to the classroom at 8:30am to transition 3 children out of the classroom.
During interviews, all interviewed staff denied they have worked alone with more than 12 preschool children. All interviewed staff stated the classroom could be out of ratio for a short period of time 5-10 minutes while they sent notification to the management staff to come to the classroom when they have more children than 1:12 ratio.
