25 | This is a follow-up proof of correction inspection conducted by Licensing Program Analyst (LPA) Tran and LPA Jung. LPAs met with Karla Barba, Director and informed the Director of the purpose of today's visit.
A review of the Facility Personnel Report Summary on 03/27/2022 indicates all facility staff or other individuals who require caregiver background checks have received criminal record and child abuse index clearances or exemptions.
Census was observed as follows: 11 children in the Older Twos classroom with staff #1 and 10 preschool children in the Twos classroom with staff #2. At 8:05am, 2 more children arrived at the Older Twos classroom; at 8:08am, another child arrived, LPAs observed staff #1 make a call to the Assistant Director to inform that staff #1 needs support to meet ratio requirements. Assistant Director sent 2 children to the Fours classroom and 1 child to the Fives classroom. LPAs also observed 9 preschool children with staff #3 in the Threes classroom, 7 preschool children in the Fours classroom with staff #4 and 12 preschool children in ,the Fives classroom with staff #5. At 9:15am, LPA Tran observed Older Twos classroom and Twos classroom including 29 preschool children were supervised by 3 fully qualified teachers and an aide in the facility playground. LPAs reviewed staff qualifications as follows: Staff #1, Staff #2, Staff #4, and Staff # 5 are fully qualified teachers; Staff #3 met requirement as a Teacher.
Facility was observed to be in compliance with staff-child ratio. LPAs observed the Notice of Site Visit was posted. There is no observed deficiency during the visit today.
Appeal Rights were explained. The Director was provided a copy of appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. First level appeals should be sent to the regional manager to the address listed above. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Director Karla Barba. (End of Report) |
