Licensing Program Analyst (LPA) Mahnaz (Nancy) Malek conducted an announced on site pre-licensing inspection at this childcare center for the purpose of opening an infant/toddler Program at this site. LPA met with the Site Supervisor, Gerardo Soto and the Program Supervisor, Silvia Magalhaes. LPA was given a tour with the representatives of the Infant/toddler classrooms (Room 17 and 18) on this combination childcare center. Room # 17 has been designated for Infant care ages 2 to 12 months old and Room # 18 has been designated for toddlers ages 12 to 24 months.

The applicant has requested to provide care and supervision for 24 infants and toddlers ages 2 to 24 months old in Rooms # 17 and 18, Monday through Friday from 7:00 AM to 6:00 PM.

Parents sign in and out at the entrance of the building leading to rooms. Upon arrival each child has an health evaluation along with hand washing before they participate in the activities.

Isolation area is the office, located at the front of the building. There will be an additional playpen with bedding for the sick child in the office. The staff bathroom will be used for sick children temporarily.
LPA discussed the posting requirements including, but limited to, the following:

Safe sleep posters / SIDS

The following was observed in the Infant/toddler rooms (Room 17 and 18)
· Age appropriate furniture and equipment’s
· Toys, tactile equipment's
· Room is well lighted with ventilation.
· Infant room has a napping area with 6 cribs not separated yet. The partition is in order.
There is sufficient open area for the child to move.
Changing table in room # 17 is placed in the classroom and within an arm's reach of a sink. There is a changing table in room # 18 inside an open bathroom area. LPA discussed the positioning of the changing tables for the staffs' visual observation of children while changing diapers. The diaper changing tables have padded surface no less than one inch tick with washable vinyl. They both have raised sides on four sides at least 3 inches high. The positions of diaper changing tables were changed for better supervision facing the rooms during inspection.
Each classroom has a baby gate in front of the doors installed.
Storage cubbies are arranged to store personal belongings
There are 6 cribs in the napping area with mattress with tight fitting sheets.
Cots are available for older children. One crib shall be used by one infant at a time only. Flooring is wood and parts of them are covered by rugs,
Bedding are provided by parents.
There is a Refrigerator in the each classroom to store the milk and other items.
LPA reminded the director that all the food shall be labeled with the child's name and date.
LPA discussed the Safe sleep practice with Site Supervisor, Gerardo Soto.

· Always place infants on their backs for sleeping
· Use only a tight-fitting sheet on the crib or play yard mattress
· Do not hang any items from the crib or above the crib
· Keep all items, including blankets, out of the crib or play yard
· Pacifiers may be used as long as they do not have items attached to them
· Infants should not be swaddled or have any items covering them while sleeping
· The temperature of the room should be comfortable enough for an adult to wear a T-shirt and not be too hot or too cold.

LPA measured the assigned Rooms (Rooms 17 and 18)
Room 17 is 590.87 square feet
Room 18 is 1042.04 square feet

Total Indoor activity space :1632.91 square feet divided by 35 = 46.65 (47 children).

The napping area which was not included in the activity area is 241.52 square feet.
Total sinks: 3 X 15= 45 children (Room 17 has one sink)
Total toilets: 2 X 15 = 30 children (Room 18 has 2 toilets and 2 sinks)
The facility has requested for 24 infants and toddlers . The infant and toddler rooms can
accommodate the required requested capacity.

Facility has designated Infant/toddler yard which is all fenced.
Outdoor activity space: 475.95 square feet which is enough for 6 children. The Site Supervisor stated they would expand it to 1040 square feet which accommodate 13.86 (14 children). LPA observed there are adequate outdoor space for expansion of the outdoor space.

· Playground is fully enclosed by an appropriate fencing, Outdoor activity space is supplied with age and size appropriate equipment, outdoor activity toys, the area will be covered with artificial grass. There is no climbing structure on this yard.

Facility has sufficient space to accommodate 24 infants and toddlers in different group and scheduling in the outdoor space. A waiver will be requested.
The Site Supervisor, Gerardo Soto is current on the required Immunization/ Pediatric CPR/ First training and has provided the completion certificate for the Preventive Health training (Nutrition and Lead component) and Mandated Child abuse reporting training.
Following items were discussed at the time of Exit interview.
Criminal Record Statement : Site Supervisor, Gerardo Soto was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Safe Sleep LPA discussed the safe sleep regulations with Site Supervisor, Gerardo Sotoand discussed the Child Care Licensing Safe Sleep webpage at
https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. The Site Supervisor was reminded of every 15 minutes check on sleeping infants under 24 months and logging them. A copy of the sample was given. LIC 9227 was discussed.
LPA also informed the Site Supervisor, Gerardo Soto of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Incidental Medical Services Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department.

A license for a total of 24 infants and toddlers, ages 2 to 24 months old in Rooms 17 and 18, Monday to Friday from 7:00 am to 6:00 pm will be issued upon receiving the following corrections and Management approval.

(1) Providing partition for infant room (Room 17) to separate the napping area from activity area.

(2) Waiver request to share the outdoor space on a staggered schedule since the outdoor space is enough for 14 children. Please include LIC 999, LIC 500, outdoor scheduling including transitioning time, and photo of the outdoor space in your request.


(3) Final picture of the infant/toddler playground after expansion.
(4) Final picture of restroom area in room 18 after being cleaned up (flooring).
(5 Providing a portable crib for isolation area when children become sick for until they are picked up.
