Licensing Program Analyst (LPA) Mahnaz (Nancy) Malek conducted an announced on site pre-licensing inspection at this childcare center for the purpose of opening an infant/toddler and Toddler Option Program at this site. LPA met with the director, Elva Tarin. LPA was given a tour with director of the Infant classroom and toddler option classroom on this combination center. One Infant room (Room # A) has been designated for Infant care ages birth to 24 months old and (Room # B) has been designated for toddlers ages 18 months to 36 months old. (Toddler Option Program).

The applicant has requested to provide care and supervision for 12 infants (birth to 24 months old) in room A and 19 toddlers ages 18 months to 36 months old in room B, Monday through Friday 6:30 AM. to 5.30 PM.

LPA observed staff following CDC and Dept of Public Health Guidelines. Due to COVID Pandemic parents shall be encouraged to follow the CDC and COVID requirements.

Parents sign in and out electronically at the entrance of the rooms. This facility is located at Walter Knott Elementary School. The elementary school is not operating on this site any longer. Therefore this childcare facility is not sharing any restrooms or playground with any other programs. The entrance is from the gate on the parking lot. Upon arrival each child has an health evaluation along with hand washing before they participate in the activities.

Isolation area is the Director’s office, located at the corner of room A of the facility and there is an additional cot with bedding for the sick child in the Director's office.

LPA discussed the posting requirements including, but limited to, the following:

· Menus / Activity Schedule

Safe sleep posters / SIDS

The following was observed in the Infant room and toddler option room (A and B)
· Age appropriate furniture and equipment’s
· Toys, tactile equipment's
· Room is well lighted with ventilation.
· Infant room has a napping area with cribs separated by small shelves. The partition is in order.
There is sufficient open area for the child to move
Changing table is placed inside one of the bathrooms in the classroom and at an arm's length of the sink, LPA discussed the positioning of the changing table for visual observation of children. The diaper changing table has padded surface no less than one inch tick with washable vinyl. It has raised sides on four sides at least 3 inches high.
Each classroom has a baby gate in front of the entrance door installed.
Storage cubbies are arranged to store personal belongings
There are 4 cribs in the napping area with mattress with tight fitting sheets. Cots are available for older children. One crib shall be used by one infant at a time only. Flooring is wood and carpeting.
All Infant items will be provided by the facility.
There is a Refrigerator in the classroom to store the milk and other items.
LPA reminded the director that all the food shall be labeled with the child's name and date.

LPA discussed the Safe sleep practice with the director, Elva Tarin.

· Always place infants on their backs for sleeping
· Use only a tight-fitting sheet on the crib or play yard mattress
· Do not hang any items from the crib or above the crib
· Keep all items, including blankets, out of the crib or play yard
· Pacifiers may be used as long as they do not have items attached to them
· Infants should not be swaddled or have any items covering them while sleeping
· The temperature of the room should be comfortable enough for an adult to wear a T-shirt and not be too hot or too cold.
LPA measured the assigned Room (Infant Room # A and B)
Room A is 553.84 square feet
Room B (Toddler Option Room) is 544.43 square feet which accommodates 15.55 (16 children)
Indoor activity space :1098 square feet divided by 35 = 31.38 (31 children).
The napping area which was not included in the activity area is 183.61 square feet.
Total sinks: 6 X 15= 90 children
Total toilets: 4 X 15 = 60 children (Room A and B have their own toilets, total of 2 in each room).
Room B for toddlers of Toddler Option Program has its own toilets for toddlers.
The facility has requested for 12 infants and 19 toddlers from Toddler Option Program. The infant room
can accommodate the required capacity but the toddler room can accommodate 16 children. Infant/toddler outdoor play yard : Facility has designated Infant/toddler yard which is all fenced to share with Toddler Option children. The following was observed:
· Playground is fully enclosed by an appropriate fencing, Outdoor activity space is supplied with age and size appropriate equipment, outdoor activity toys, the area is covered with grass. There is one play structure on the wood chip cushioning. The facility will send a waiver request to share the playground with Toddler Option children on different scheduling.
Outdoor activity space: 12730 square feet divided by 75 = 169.73 (170 children)

Facility has sufficient space to accommodate 12 Infants and 16 toddlers from toddler option program in the designated Infant yard.

Fire clearance received from Orange County Fire Authority approving the requested capacity of 12 infants in room A and 16 toddlers in room B. The director was reminded the maximum group of children in the toddler option program is 12 with 2 staff. If this number exceeds another group shall be created separate from the other group.

The director, Elva Tarin is current on the required Immunization/ Pediatric CPR/ First training and has provided the completion certificate for the Preventive Health training (Nutrition and Lead component) and Mandated Child abuse reporting training.
Following items were discussed at the time of Exit interview.
Criminal Record Statement : The Director, Elva Tarin was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
Safe Sleep LPA discussed the safe sleep regulations with director, Elva Tarin and discussed the Child Care Licensing Safe Sleep webpage at
https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed the director, Elva Tarin of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Incidental Medical Services Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department.

(1) Providing partition for infant room (Room A) to separate the napping area from activity area.
(2) Waiver request to share the outdoor space with toddler option on different scheduling. Please include LIC 999, LIC 500, outdoor scheduling, and photo of the outdoor space in your request.
(3) Amendment for Toddler Option Program parent handbook regarding parent's consent letter if transitioning to preschool before the age of 36 months.
