LPA unable to interview the RP for further information due to contact information not being provided.
During the investigation LPA Lucero interviewed 5 staff, 4 parents, obtained documents, and reviewed the facility roster.
During staff interviews, 3 out of 5 staff members disclosed the facility has been out of ratio. No other staff made disclosures.
LPA attempted to interview 7 parents, however only 4 parents were available for interview. Parents interviewed expressed no concerns.
Based on LPA’s interviews conducted with staff, it has been determined the facility has operated out of ratio. Therefore, the preponderance of evidence standard has been met, therefore the above allegation is found to be Substantiated. California Code of Regulations, Title 22, 101216.3(c)(1)(C) Teacher-Child Ratio is being cited on the attached LIC 9099D.
LPA Giselle Lucero informed Director Jennifer Arreola, that this report dated 11/20/2024, documents 1 Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care. Also, LPA Giselle Lucero informed Director Jennifer Arreola, to provide a copy of this licensing report dated 11/20/2024 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
Exit interview conducted and report was reviewed with the Director Jennifer Arreola. A notice of site visit was given and must remain posted for 30 days.
Appeal Rights were explained. The Director was provided a copy of appeal rights (LIC 9058 01/16) and their signature on this form acknowledges receipt of these rights. All appeals must be in writing and received by the Regional Office within 15 business days. First level appeals should be sent to the regional manager to the address listed above.
